  1. All right, So this is going to stand as the
  2. third and final check before your patient receives this medication.
  3. So you need to look at the Mar.
  4. You need to look at the rate of administration for
  5. this, so keep in mind that you're gonna have to
  6. program the pump.
  7. According Thio, the rate that your patient needs to receive
  8. this medications, the pumps work in a miles per hour.
  9. So if it's not in this, you need to convert
  10. it into over hours instead of if it's 1/2 hour
  11. or how long it's gonna take.
  12. You need to be able to calculate that.
  13. Um So, like I said before, um, the primary and
  14. secondary have already been prepared Prepared that at the beginning?
  15. Um, so let's just start with the primary.
  16. So, like I said, you're going to check your Mar.
  17. Um, this is the third time you're checking.
  18. You're gonna program the infusion device that you're gonna be
  19. using the pumper, the facility's policies.
  20. You're going to use aseptic technique to scrub the port
  21. of the extension set.
  22. Scrub it for at least 10 seconds on to make
  23. sure you're getting everything off of the extension.
  24. set before you actually hook up.
  25. That medication s so you're gonna feed the medication, The
  26. tubing all the way through the pump.
  27. Um, there's gonna be a little portion where you actually
  28. push in one of the clamps.
  29. Um, in order to secure the line in there, it's
  30. easier. Hands on showing.
  31. Um, but you're gonna feed the medication through the pump
  32. and then connect the medication again.
  33. You've already made sure that there's no bubbles.
  34. There's no like air bubbles.
  35. If you did, you would tap them out prior to
  36. connecting this.
  37. You've already scrubbed it.
  38. Um, you've already verified your Mars.
  39. You're gonna follow the prompts on the device to start
  40. the infusion of the medication.
  41. Now we have this secondary medication again.
  42. We've prepared this already.
  43. There's no air bubbles, but if they're waas, you would
  44. tap to remove them.
  45. You're using that secondary to being, um you've already verified
  46. Jamaar. This would be the third check for this medication.
  47. You verified.
  48. All the medications are not expired.
  49. The packaging was intact.
  50. You've done all of that.
  51. Um, so you're going to connect to the tubing to
  52. the port above the pump.
  53. So as I put in this little picture here, That
  54. is the first port of the administration set.
  55. You are going to scrub that for 10 seconds before
  56. you're actually attaching.
  57. Um, before you're attaching the secondary home set to this
  58. primary tubing.
  59. So scrub scrub, scrub for 10 seconds, then you're going
  60. to screw the port on when you hang the bag.
  61. It's gonna be above the primary medication.
  62. I have a picture in the next life to show
  63. you this, but it's that's actually going to be above
  64. it. And then you're gonna, um, the pumps already program
  65. like me did the primary set.
  66. If you had Thio program it again, you would follow
  67. the facilities policy, follow the poem on, do that and
  68. then you want to make sure all of your two
  69. beings are labeled your initial in your date time.
  70. Make sure everything is labeled.
  71. Everything's capped up any any ports.
  72. You don't want to leave them open because they can
  73. get contaminated, so make sure everything is sterile and we're
  74. not giving any infection to our patient.
  75. So now let's see what that secondary medication actually looks
  76. like. So, as you can see here, you have the
  77. primary set that hangs a little bit lower than that
  78. piggyback secondary set on.
  79. And it attaches to that first port off of the
  80. primary set.
  81. So just to give you a little visual of how
  82. that's gonna work So now that we went over the
  83. primary and secondary medications, let's say, for instance, we have
  84. an I V push via a saline lock.
  85. So for this, you're going to recheck the Mar again.
  86. You're going Thio, drop your medication just like we did
  87. in our prior medication are check off using sterile technique
  88. using a blunt tip Thio draw from the syringe, making
  89. sure that you're cleaning the vials prior toe drawing up
  90. the medication.
  91. And then once you've verified you've gathered your medication, you're
  92. going to use alcohol Swab Thio clean the port of
  93. the extension said again for at least 10 seconds, you're
  94. gonna flush the ivy line to make sure it's still
  95. good. It's not like infiltrated.
  96. Nothing's wrong with it.
  97. Then you're gonna actually attach the syringe of medication to
  98. the ivy.
  99. Poor again.
  100. Make sure you're not touching the end of the syringe.
  101. You want to make sure it stays clean, stay sterile,
  102. not clean.
  103. sterile. Um, And then you're gonna inject the medication, her
  104. the orders.
  105. So you don't wantto too fast.
  106. You wanna push too slow?
  107. You want to go based off of the orders and
  108. based off of the medication, push it at the appropriate
  109. rate. And then once you're done administering this medication, you
  110. want to flush the line.
  111. You wanna make sure the patients getting the entire dose
  112. of the medication.
  113. You're not keeping something in the extension set, and it's
  114. not affecting the patient.
  115. So make sure you flush it out and then capped
  116. the end again.
  117. Clean it, then cap it, make sure nothing gets introduced
  118. into their.
  119. So if you're giving an I V push for a
  120. continuous infusion, so for this, you're gonna place the pump
  121. on hold.
  122. Stop it for a couple minutes while you give this
  123. medication you're gonna clean the injection poor of the same
  124. lock again for 10 seconds.
  125. This is a constant.
  126. You're always cleaning that you don't want to get bacteria.
  127. Insert it right into the ivy, and then you're causing
  128. an infection.
  129. Um, you're gonna flush the line out again, making sure
  130. that it's intact.
  131. It's running, it's flowing there's nothing wrong, No kings, anything
  132. on. And then from there you can attach the syringe
  133. just like we did in the prior slide.
  134. Like the I V push.
  135. I've seen, um, where I've seen where some nurses actually
  136. clamp the other end above where they're pushing to make
  137. sure that the medication does not backflow to make sure
  138. it's actually going down the line to make sure that
  139. the patient's getting all the medications.
  140. You can also do that so attached the syringe.
  141. You'll adjust the medication.
  142. Um, per the push rate you're not giving too slow,
  143. Not too fast per how the medication is recommended to
  144. be given.
  145. And then you're gonna flush the line, um, so making
  146. sure your patient gets all of that medication and then
  147. restarting the infusion as ordered.
  148. Um, so with all of this, who would go based
  149. off of your provider's orders and continue everything that was
  150. already ordered?
  151. All right, so with every medication that we give Ivy
  152. we're gonna flush After that medication was administered before you
  153. attach the flush cleaning for the 10 seconds that's gonna
  154. be something you do every single time.
  155. You don't wanna have an infection or make anything worse
  156. for your patient, so make sure your patient gets all
  157. that medication and flush it out.
  158. All right, so now your patient got all the medications.
  159. They got your primary and the secondary medication in the
  160. I V push medication.
  161. Now you see an order come through for the removal
  162. of the Ivy catheter.
  163. The first thing you want to do is verify your
  164. patient's name and birthday and verify there's an order for
  165. the removal.
  166. You do not want to realize later that you took
  167. it out of the wrong patient because somebody might be
  168. mad if that was a very hard stick.
  169. Eso verify that.
  170. Wash your hands, apply gloves just like we did for
  171. the Ivy.
  172. You're gonna wear your gloves.
  173. Standard precautions.
  174. Protect yourself and the patient.
  175. Want to contaminate anything?
  176. Gather your equipment to buy two gauze and tape Discontinue
  177. All too big right for the provider provider's orders.
  178. There shouldn't be any medications left if your provider ordered
  179. it to be removed.
  180. Removal dressing from the catheter, the extensions that remove everything
  181. on and then remove the ivy catheter and apply pressure
  182. with gauze.
  183. Um, so once you do that you can secure it
  184. with some tape.
  185. Make sure patient doesn't have an allergy to the tape
  186. before you put it on them on.
  187. And then inspect the ivy catheter for in Tekken issue.
  188. Want to see if any portion is broken off or
  189. anything that should be concerning for you.
  190. So monitor that also you're gonna want a document.
  191. So just like we documented that it was put in
  192. place. We're also going to document the removal of it.
  193. Just put what kind of tape you put on.
  194. Um, patient tolerated.
  195. Make sure that you do that as well.
  196. So that we know that it was removing patient didn't
  197. go home with an ivy.
  198. Next is the closing and exit.
  199. So we have incident and ivy.
  200. We've given all of our medications, and we have removed
  201. the ivy.
  202. So now we're gonna be closing.
  203. So we are going to place the bed in the
  204. lowest position, but the side rails up.
  205. We're gonna hand the patient to call light and say
  206. Okay, if you need anything, you can push that red
  207. button at the top.
  208. I can come around or a CNN can come around.
  209. Um, but I should be around every off every hour
  210. to come check on you.
  211. But if you need anything else, I am here and
  212. again. We're gonna remove our clubs to purchase checked ourselves
  213. or without contaminating ourselves.
  214. So we're gonna follow, though, Um, those special procedures were
  215. gonna wash your hands and perform hand hygiene on dhe.
  216. Then again, tell your patient, see if they need anything
  217. before you leave.
  218. You're there for them.
  219. And if they have any more questions, feel free to
  220. answer them.
  221. And that concludes wth e ivy Insertion Medication administration and
  222. removal of an I V.
