  1. Hi, my name is terry in Green and I'm a
  2. Doctorate of Nursing DNP student at Brandon University School of
  3. Nursing and a student and Professor Weber's D.
  4. M. P.
  5. U 706 class.
  6. I'm presenting on my stakeholder education plan for my DMP
  7. capstone project.
  8. I would like to start off with the pick a
  9. format to explain my project.
  10. Our project will look at p primary care providers intervention
  11. would be the use of a tele health providers seek
  12. compared to a standard in person provider.
  13. Oh, the outcome is to improve staff, job stress and
  14. job dissatisfaction over tea time, Which is a three month
  15. period. Primary care is most often the first point of
  16. entry for most patients entering in the medical care.
  17. Health care providers shortages have existed for decades and colleges
  18. and programs have not been graduating enough mds mps, mps
  19. to meet the provider demands of health care even more
  20. in primary care.
  21. These shortages have been further compounded upon during the pandemic
  22. telehealth has existed for some time, but with somewhat underutilized.
  23. Prior to the covid 19 pandemic.
  24. The pandemic somewhat pushed for the use of telehealth and
  25. somewhat fourth healthcare entities to use telehealth to provide health
  26. care to patients from a distance.
  27. During the pandemic.
  28. During the pandemic, government entities loosen laws and practices started
  29. adopting telehealth, offering telemedicine e health ari visits more often
  30. called telehealth.
  31. In the use of telehealth exploded during the pandemic and
  32. it was found, telehealth visits increased by as much as
  33. 154% during the last week of March 2020 of the
  34. pandemic telehealth has not become a regular route for providers
  35. to provide health care and patients to receive care.
  36. In this matter, telehealth has been pushed to the forefront
  37. since the pandemic and it could possibly serve as a
  38. way to also reduce the ever growing shortages of health
  39. care providers in primary care and to also expand patient
  40. access to care.
  41. My project looks at how the use of telehealth could
  42. benefit and are transformed primary care as a primary care
  43. mp. I know this while working in primary care, there
  44. were shortages of primary care providers which cost providers to
  45. have to cover more patients and providers feeling overwhelmed because
  46. of increased patient loads and increased responsibilities.
  47. This caused job stress and job dissatisfaction and resulted in
  48. some providers leaving primary care.
  49. I eat a high turnover rate in primary care.
  50. These high turnovers further creative cycle where remaining providers became
  51. even more overwhelmed because it was hard to covering more
  52. patients from the providers who left and which further increased
  53. patient loads, job stress and job dissatisfaction.
  54. And then more providers leaving primary care.
  55. I decided to take the current problem I reserved in
  56. practice to base my D.
  57. M. P.
  58. Project upon and towards finding a way interventional solution that
  59. could possibly change the cycle of this job stress, job
  60. dissatisfaction and high turnover in primary care.
  61. Further benefits and a solution to this problem could also
  62. include expanding care for Peyton.
  63. My project focuses on primary care practices and small clinics
  64. are proposed projects includes employing a part time primary care
  65. and P to provide telehealth to patients.
  66. The intervention is projected to decrease patient workloads for on
  67. site primary care providers and which could reduce their provider
  68. job stress and job dissatisfaction.
  69. Other benefits projected are decreased job stress and decreased job
  70. dissatisfaction, as well as a reduced burnout and high turnover
  71. in primary care.
  72. It's also been anticipated through adding a tele health provider.
  73. This could also increase patients, decrease patient employment, wait times,
  74. increased reimbursements and in turn increase revenues.
  75. Ultimately. Long term benefits include increased availability of providers and
  76. expanded access to care.
  77. Current models for how primary care practices and clinics function
  78. include commonly having employed full time or part time providers,
  79. mds mps, mps who drive to locations to provide on
  80. site primary care services to patients who also drive to
  81. locations to receive care from the providers.
  82. The model I plan to introduce includes employing a part
  83. time telehealth provider to everyone Onside primary care provider, The
  84. on site provider will focus on doing acute care patients
  85. while the tele health provider will focus on chronic care
  86. patients know an acute care patients often need hands on
  87. physical assessments, labs and diagnostics.
  88. So there will be more for suitable for on site
  89. primary care visits.
  90. While chronic care patients who usually are established patients who
  91. have more predictive bull diagnoses, symptoms and medications and not
  92. as often needing to be seen on site would be
  93. more suitable for telehealth visits.
  94. The premise for telehealth providers focusing on chronic care patients
  95. can give on site providers more time to see new
  96. and acute care patients and which could possibly decrease their
  97. level of job stress and job dissatisfaction.
  98. This in turn may hopefully decrease providers desires for wanting
  99. to leave primary care because they are not as stressed
  100. or dissatisfied with their jobs because it's been shown an
  101. increased stress and increased job dissatisfaction can be correlated with
  102. high turnover rates in primary care.
  103. Other possible benefits could be that decrease healthcare costs because
  104. there are no cost for providers or patients to travel
  105. to the physical sites as well as patients being seen
  106. in telehealth.
  107. They have lower incidences of no shows because they will
  108. be being seen at their home and have not have
  109. transportation as a barrier.
  110. The theoretical framework are planning uses the human Factor Systems
  111. Engineering Initiative for patients safety S.
  112. C. I.
  113. P. S.
  114. And extended mono 2.0 and then then which introduces And
  115. incorporates three noble concepts, configuration and engagement and adaption.
  116. The concept of configuration highlights the dynamic, hierarchical and interactive
  117. properties of social technicals.
  118. Systems, making it possible to depict how health related performances
  119. shaped at a moment in time, engagement conveys that various
  120. individuals and teams can perform health related activities separately and
  121. collab laboratory Vly engage individuals often include patients, families, caregivers
  122. and other professionals and nonprofessionals.
  123. Adaptation is introduced as a feedback mechanism that explains how
  124. dynamic systems evolved and planned and unplanned ways.
  125. Conceptual models I plan to include are the stress and
  126. burnout conceptual model for providers and the telemedicine conceptual model
  127. for patients.
  128. Concepts include job stress, job dissatisfaction, emotional exhaustion, depersonalization and
  129. personal accomplishment and providers through like a 10 item burnout
  130. measure short version BMS.
  131. Or five items short index of job satisfaction S.
  132. I. D.
  133. S. Adopted to determine job satisfaction dissatisfaction, right and burn
  134. out. And providers tell them that the telemedicine conceptual model
  135. to evaluate health care, professional satisfaction and utilising telemedicine and
  136. telehealth can also be used and including looking at five
  137. components. Then if you then influence health care professionals, perceptions
  138. and satisfaction with telehealth as followed one demographics to The
  139. care setting.
  140. three the providers experience was telehealth for motivation which includes
  141. patient costs in time and five.
  142. The oversight all satisfaction of the provider with the telehealth.
  143. The telemedicine conceptual framework can also be used for assessing
  144. patient satisfaction with the telehealth visits.
  145. It may include surveys or questionnaires such as the telemedicine
  146. satisfaction questionnaire and or the telemedicine satisfaction and youthfulness usefulness
  147. questionnaire stakeholders from my project include private primary care practices,
  148. small clinic owners, collaborative physicians in primary care and hospital
  149. own clinics and practices.
  150. MD own clinics or practices mps who own their own
  151. clinics and practices primary care providers, primary care colleagues who
  152. may be willing to adopt the same model tell how
  153. platform developers and companies as well as patients caregivers the
  154. community. My university chair, university program director and I.
  155. R. B.
  156. Panels. The setting for my project is a private primary
  157. care Mp own clinic in a metropolitan area.
  158. The clinic is owned by Mp, who has been a
  159. friend and mentor to me over the last few years
  160. and whom I previously worked for as A.
  161. F. And P.
  162. At her clinic.
  163. The project has already been introduced and accepted by the
  164. Mp clinic owner and it is anticipated.
  165. I will send her a follow up video such as
  166. this one to further introduce the plan for the project,
  167. as well as a cost benefit analysis in the outline
  168. for how the project will proceed.
  169. Primary care clinic or practice owners, providers and colleagues would
  170. benefit from my project and that it can provide them
  171. a model of evidence for why adopting this model of
  172. care could help to increase their practice or clinic patient
  173. lows, decrease their provider stress and job dissatisfaction and possibly
  174. decrease provider turnover in their primary practices and clinics as
  175. well as expanding, increased patient access to care.
  176. As long as as well as increased revenues, cost, benefits
  177. have already been projected and benefits of the project have
  178. been found to outweigh the costs And so show feasibility
  179. for the project as personal as well.
  180. In all 2020 cost projected included in the project include
  181. a projected salary for telehealth provider, cost for Internet, laptop
  182. or computer will be at no cost because it would
  183. be provided by the tele health provider.
  184. There will be cause for registration for the EMR which
  185. is practice fusion but the telehealth platform will be at
  186. no cost because it is included in the practice fusion
  187. EMR. Other benefits would include, there would be no cost
  188. for transportation for both providers and patients who participate in
  189. telehealth visits as well as other benefits.
  190. Maybe adding a telescope provider could include an increase in
  191. patients and with two providers there can be seen more
  192. patients than one provider and increased patients equal increased reimbursement
  193. and increased reimbursements equals increased revenues.
  194. Part of the process of my project includes setting criteria
  195. for the project patient participants as well as identifying my
  196. sample and my population as well as explaining to the
  197. patients the project gating informed consent as then moving on
  198. from there to start the project.
  199. Once I've identified my criteria, sample and population.
  200. My project includes me as the DNP student to work
  201. as a part time to help provide are employed to
  202. the practice or clinic along with the Mp owner or
  203. another Mp who will work on the on site as
  204. an on site Mp provider.
  205. The current M.
  206. R. Platform will be used by the clinic is practice
  207. fusion and I would be added to the M.
  208. R. Platform to be able to access patient charts as
  209. in diagnostics into chart practice fusion has a built in
  210. telehealth platform which I will be using for free and
  211. which I will be seeing the patients via telehealth once
  212. I have received informed consent and participants are willing to
  213. participate. I will then establish a telehealth link through the
  214. EMR telehealth platform and where patients can access the visit,
  215. patients will willing to participate in project Project will then
  216. be assessed for their availability, appointments and arrange appointments in
  217. the EMR.
  218. The patients will then be also assessed for if they
  219. have access to the internet or an internet enabled phone,
  220. computer, laptop or tablet was camera capability and so they
  221. can access to telehealth platform for e visits with me
  222. as the tele health provider if video will also be
  223. made and by me which will describe the project and
  224. how the patient can access to telehealth platform and it
  225. will be sent to the patient's phone or their email
  226. instructing their patient how to access and use the telehealth
  227. platform. Surveys will also be used in the project and
  228. based off the human factors framework and stress and burnout
  229. and telemedicine conceptual models.
  230. During the middle of the project, patients will be asked
  231. about their experiences and their satisfaction with the telehealth visits
  232. per like er scale type survey as well.
  233. Clinic staff and providers will also be given surveys at
  234. the beginning and in the middle of the project.
  235. Once I reach my sample and population I will then
  236. start the project throughout the project.
  237. There will also be mid points where there will be
  238. meetings between staff and also surveys as per as per
  239. indicated before.
  240. This is where U.
  241. S. Stakeholders are important.
  242. Your feedback and guidance And ideas and opinions are needed.
  243. You will be needed before at the beginning during and
  244. at the end of the project.
  245. Your feedback opinions and ideas are needed to help guide
  246. this project towards being successful.
  247. Your feedback opinions and ideas will be via short online
  248. surveys or 2-3 short online Zoom meetings which will be
  249. before the project at the start of the project and
  250. at the end of the project.
  251. Also shareholders will be informed about the project's progress via
  252. email progress reports, including information for how many patients have
  253. been seen by the on site intel health providers.
  254. Each week, reports will also include feedback from meetings with
  255. the on site and pre provided the telehealth mp provider
  256. and office staff throughout the project.
  257. That will be logged into AMS word spreadsheet and once
  258. the project is completed, all data received from the project
  259. will be reviewed and confirmed for for the project.
  260. The M.
  261. S were expressed.
  262. The data will then be reviewed and analyzed and then
  263. graft an interpretative into statistics and any in for instance,
  264. is made to deterrent feasibility of the project and if
  265. the interventions made significant changes, statistics developed developed from the
  266. data along with all the data and fighting's, will then
  267. be included in the proposal, written up about the feasibility
  268. and success of the project, hopes, are that the project
  269. and the data and findings found from the interventions in
  270. the project will demonstrate reason for the intervention and here
  271. on my references.
