  1. Hi. My name's Autumn Slash and are representing this case
  2. study, which will briefly look at the road to improve
  3. dental outcomes for a pediatric patient through evidence based behavioural
  4. management and preventative advice.
  5. A quick overview of our patient.
  6. His name's Andy is 14 years old.
  7. He's got traumatised upper right one.
  8. He's got autism and poor oral health with Kerry's present
  9. in all quadrants.
  10. His social history involves being in a regular attendee at
  11. the dentist, and he's in temporary foster care.
  12. So Andy fits the British Society of Pediatric Dentistry definition
  13. on dental neglect.
  14. He also fits the description that they put forward, which
  15. describes the most vulnerable Children to not attend appointments.
  16. So the Barts Health Special Care Community Dental Service recommended
  17. in their audit reviewing missed appointments by Children to key
  18. areas, which, which could be improved it.
  19. It suggested that there should be an increased engagement with
  20. parents and that dentists should use this opportunity to really
  21. understand a carers perceptions, willingness or maybe the lack of
  22. However, the B S P D do want dentists not
  23. to instantly assume that just because a child has dental
  24. neglect or doesn't show up to their appointments that they
  25. are being abused, and the dentist should be empathetic with
  26. the carers and try and identify barriers and overcome them.
  27. And with regards to Andy's case, I think the dentist
  28. should be cautious that he has had to trauma incidents
  29. to his front teeth in the past six months and
  30. just notice.
  31. Just be aware and vigilant with regard to any other
  32. signs of trauma on the patient's face body and in
  33. their demeanor as well.
  34. And diesel diet and lifestyles Quite poor.
  35. He's got a limited high sugar diet.
  36. Any smokes around 10 a day, which is quite alarming
  37. for his age.
  38. And as we know, the consequences of these factors are
  39. bad breath, gum disease and an increased risk of oral
  40. cancer. With regards to a smoking and the behavioral management
  41. of this, the BMJ released a paper which did highlight
  42. that there is an increased prevalence of teen smokers in
  43. recent years and that there's a low probability that they
  44. will stop on their own without formal intervention.
  45. A study by Brown showed that there's a positive effect
  46. of motivational interviews for smoking cessation in adolescents with psychiatric
  47. disorders. So and this treatment plan consisted of prevention, appointments
  48. and other appointments to deal with these other dental issues.
  49. During the prevention clinic, there are three key areas which
  50. are dentists must visit.
  51. So he dentist must assess the education, the family health
  52. or the social issues around the patient and the carers,
  53. and also understand that individuals have differing life priorities and
  54. understand what they are with regards to Andy's carer and
  55. make them aware that then dental health is very important
  56. to a child's overall well being and that a child
  57. has complex and that the carer might have child complex
  58. childcare arrangements, as Andy does have five younger siblings also
  59. defining needs and tailoring preventative care and treatment to the
  60. situation as it is at present.
  61. And also consider contacting all the professionals, such as the
  62. social worker or support worker, to support the carer and
  63. Andy encourage them to meet their dental appointments.
  64. Specific carries prevention could involve personalized oral health promotion advice,
  65. and this can be hands on brushing instructions for Andy
  66. to help him visualize and understand what's required of him
  67. When brushing a prescription for a 2800 ppm, fluoride toothpaste
  68. can be given a detailed diet advice with specific suggestions,
  69. Um, such as alternative snacks and alternative meals and when
  70. they should have them officious ceiling and a floor I
  71. varnish can also be applied.
  72. So some special considerations with regards to Andy's case is
  73. that he's got autism and there might be a lack
  74. of understanding.
  75. And there might not be, um, you'd have to assess
  76. whether he was a high funk, whether he was high
  77. functioning or low functioning, and he might most probably have
  78. sensory issues as well.
  79. He's a young teenager, and therefore he'll have a desire
  80. to be more in control and independent, which could involve
  81. him telling the dentist when to stop and start drawing
  82. procedures. Also, he's got poor oral health in general due
  83. to his infrequent brushing poor diet and smoking.
  84. So use images and photographs to to aid understanding of
  85. the procedures and also allow the patient to hold objects
  86. to distract them.
  87. Drawing treatment can really benefit compliance.
  88. Also, sound canceling earmuffs have been used in autistic patients
  89. before to block out loud drilling and avoid, um, sort
  90. of an anxiety trigger in the patient.
  91. So here we have a route map of health behavior
  92. change using motivational interviewing, and this can really aid overcoming
  93. bad behaviors such as smoking and poor diet.
  94. And then this involves using sores to gain empathy with
  95. the patient and really illicit change talk.
  96. Then there can be an educational intervention and improving knowledge
  97. and skills, and then an action plan with a set
  98. time, date and place to start and also encouraging habit
  99. formation and repeat all of this at every recall visit.
  100. Thank you for listening to my presentation.
