  1. Bye, everyone.
  2. My name is Tony Hand.
  3. The agency I am at is with promise of behavioral
  4. health. This is for field to Dr Watkins, and here's
  5. my case presentation, So promise it provides several service's we
  6. have provide adoption.
  7. Service is they start with the family from the beginning.
  8. They are there 24 hours, seven hours, seven days a
  9. week, providing anything that the family may need.
  10. The child's in crisis.
  11. If another member of the family is in crisis due
  12. to the adoption, they're there to walk the family through
  13. that process.
  14. They also provide strip, which is the short term residential
  15. therapeutic programs.
  16. They are for adolescence or youth from the ages of
  17. 12 to 17 years of age.
  18. Of course, we provide them a shelter.
  19. They do get therapy.
  20. We have about nine homes that are owned by Promesa
  21. in Fresno.
  22. Um, we have two homes.
  23. One is particularly for, um, youth that are pregnant or
  24. mothering, so they get that certain kind of help they
  25. need for the power that they are on.
  26. We also our offer a sex offender program, which we
  27. have a house that is dedicated, um, to the needs
  28. of the youth in that program foster care.
  29. They offer all training pertaining to foster care.
  30. The help with certification.
  31. They give you a bonus because, of course, like everywhere
  32. else, we are in very much in need of foster
  33. families. They provide respite care to give, um, the family's
  34. a break if needed.
  35. And again they offered the 24 hour, seven day a
  36. week support.
  37. We do have a substance abuse abuse program.
  38. It is outpatient for 18 and up.
  39. The are provided therapy into modalities, which is group therapy
  40. and individual therapy.
  41. I thought the nice thing about this program, which I'm
  42. sure it is in other programs as well they take
  43. medical. And then if there are fees over what medical
  44. can pay, they're not going to charge the clients.
  45. They're set up with contracts from other businesses and places
  46. in Fresno that are willing to fund on the excess
  47. of what medical cannot pay.
  48. And, of course, if they don't qualify for that, there's
  49. a sliding scale.
  50. They re receive, um, 2 to 5 times a week,
  51. 90 minutes worth of therapy again, either individual or group.
  52. So moving on to the client, we will call her
  53. Mickey. She is an 18 year old female of Mexican
  54. American descent.
  55. She's only been in the system for abouts two years,
  56. and she was 16.
  57. And she does have a complex trauma.
  58. Her background was her mother.
  59. I did drugs for as long as she could remember.
  60. She went from her mother's house to a grandparent's house
  61. all the way.
  62. Um, from the earliest she could remember to the age
  63. of 16.
  64. There was many times that she had an open case
  65. with CBS, but, um, before being 16 and actually being
  66. taken from the home, nothing ever came of that.
  67. She did say that her father only came around when
  68. he wanted to sexually abuse her.
  69. And the grand parents that she was living with were
  70. the father's parents.
  71. Um, so finally, she was taken from the home, and,
  72. um, she is now with prom SS.
  73. So the goals and three assessment that we have done
  74. for Mickey she struggles with depression, um, anxiety, time management
  75. and risky behavior.
  76. I am unsure of the diagnoses that Prom Essa has
  77. given her at this weight.
  78. I know that she does have an I p.
  79. She's in special education she's has a processing disorder and
  80. a d d.
  81. So that time management, um, probably comes from the a
  82. t d.
  83. She is.
  84. She follows all the rules at the home.
  85. She's has her moments in deep depression where she will
  86. try to a wall.
  87. But since she's been with promised, uh, she has not
  88. a world, so that is a great improvement for her.
  89. She is a senior, so her goals as she wants
  90. to graduate, um, control emotions learned coping skills for her
  91. depression, and her goal is to prepare for Slip, which
  92. is where the social worker work will place her in
  93. a more independent home, either alone or with a roommate.
  94. At this point, she's not quite ready to be part
  95. of slip, but she is almost there.
  96. So we're excited to see her make improvements in that
  97. way so that most of the treatment that she has
  98. is dialectical behavior therapy.
  99. I'm d B t, which was first, um, brought about
  100. to help people with the borderline personality disorder, but they
  101. have found that it works very well.
  102. Um, with any almost any diagnosis.
  103. So, um, the four main parts of DVT is the
  104. emotional regulation mindfulness to stress tall, a tolerance and interpersonal
  105. effectiveness. She did say that Mickey did say that she
  106. likes her therapy program.
  107. She likes her clinician.
  108. Um, I have been involved with more of the mindfulness
  109. part, Um, at with Mickey.
  110. So the social work skills that I have done with
  111. this particular client is, um, pre engagement.
  112. That's just getting to know her.
  113. Ah, lot of times these kids are, I should say
  114. youth are in and out of these homes and in
  115. and out of foster care.
  116. And, um, there they're coolant.
  117. Clinicians change often, which is unfortunate as well as their
  118. skills group leaders.
  119. They often change.
  120. So the pre engagement is pretty important.
  121. I have to or I had to just spend a
  122. lot of time just being in the home, not trying
  123. to, um, offer any service is just being there and
  124. getting to know the girls engagement.
  125. I have been a part of the actual engagement with
  126. this client getting her to commit two, um, parts of
  127. the therapy.
  128. Um, she didn't like mindfulness.
  129. And so, um, after the pre engagement and talking with
  130. her, she finally decided to be part of the D
  131. B D group when we to mindfulness.
  132. So that was also, um, very good to see her
  133. come into that I'm And then, of course, the skills,
  134. the skills groups, Um and that's where we That's what
  135. I would do our therapy with them, um, as well
  136. as the mindfulness.
  137. So case presentation.
  138. Why? What does this have to do?
  139. A social work.
  140. So I during our supervision, I noticed that each week
  141. there's a coalition that will do a case presentation.
  142. So it's important to be able to lay out a
  143. presentation so that you can get sufficient supervision.
  144. The supervisor, the psychologist, cannot help if they don't know
  145. the background of the client, um, organizational skills being able
  146. to provide all of the information in one spot.
  147. Of course, that is also sometimes we use thes skills
  148. and i p meetings for schools.
  149. I have seen them done, so you need to explain
  150. to the teacher two the principal.
  151. Whoever needs to hear about the client, the case presentation
  152. is a good way to provide all of that and
  153. court procedures.
  154. Months, you're on that one, but you may need toe
  155. provide a case presentation if if um something needs to
  156. be heard in the court case.
  157. So that is Mike case presentation on Mickey.
  158. Yeah, and thank you for listening, and I will now
  159. take questions.
