  1. Hello, This is Allison Burgsmueller.
  2. I'm a family nurse practitioner.
  3. I'll be presenting increasing identification of social and emotional developmental
  4. delays in Children.
  5. The purpose of the presentation is to inform the community
  6. on how to integrate and evidence based screening tool into
  7. the primary pediatric healthcare setting, which would increase the identification
  8. of social and emotional developmental delays in Children.
  9. In San Luis Obispo County, the focus is to educate
  10. health care and early education professionals by presenting the ages
  11. and stages for social emotional questionnaire, also known as the
  12. Sqs E, which has proven psychometric properties at increasing identification
  13. of social and emotional deficits in Children 2.5 to 5
  14. years of age.
  15. The American Academy of Pediatrics recommends health care providers uses
  16. screening tool for all developmental domains, including fine motor, gross
  17. motor communication, self help and problem solving.
  18. A screening tool that can be used for all developmental
  19. delays is the ages and stages Question Ear, also known
  20. as the S Q three, but does not increase identification
  21. of social emotional deficits.
  22. An estimated 5 to 20% of all preschool aged Children
  23. are affected by social and emotional delays, since there is
  24. no recommended screening tool being consistently used in the clinic
  25. setting. Many Children are not identified until they enter the
  26. education system at kindergarten.
  27. The significance of the problem is less than half of
  28. Children with a positive A s QSC score are identified
  29. when screen solely with the S Q three.
  30. Many broad spectrum screening tools lack psychometric properties specifically when
  31. screening for social and emotional delays.
  32. Therefore, many Children go undiagnosed, including behavioral, social, emotional, autism,
  33. spectrum disorder, motor specific sensory and psycho social conditions.
  34. Okay, First five San Luis Obispo is organizing Entity of
  35. the Help Me Grow Committee, which acts as the centralized
  36. access point for pediatric outreach.
  37. Community outreach and data for early childhood service is in
  38. the community.
  39. Pediatric healthcare providers are the first opportunity to survey and
  40. screen for social emotional delays because of the repeat exposures
  41. at early childhood well and sick visits, Children with social
  42. emotional deficits are more responsive to early intervention.
  43. Service is due to the brain's rapid growth and pliability
  44. in early childhood before the before kindergarten, making early education
  45. educators critical factors and identifying social and emotional deficits.
  46. The S qsc can be completed by a caregiver in
  47. this highly reliable screening tool that focuses on social emotional
  48. development and Children accurately identifying social emotional developmental early on
  49. paves the way for family education and further assessment interventions,
  50. which helped Children reach their full potential during their most
  51. informative years.
  52. The S Q S E is easy to use, cost
  53. effective caregiver completed and culturally sensitive tool results can be
  54. scored quickly and identifies.
  55. Children at risk, however, also addresses any other questions or
  56. concerns a care giver might have and want to talk
  57. with the health care provider.
  58. The seven areas of the sq include child's autonomy, compliance,
  59. adaptive functioning, self regulation, affect interaction and social communication.
  60. An extensive literature review of several databases, including Signal Cochran,
  61. Pub Med and Psych Info, focuses on research published in
  62. the last five years.
  63. Written in English, the search yielded high return with synonyms,
  64. used young Children, ace QSC, developmental screening and developmental delays.
  65. A core concept imperative to the synthesis of research reviewed
  66. is age interval, where the sq is most effective.
  67. The interval consistency of three studies concluded Kaka Alfa at
  68. 30.70 in ages 30 to 60 months, indicating high reliability
  69. whereas ages six months to two years less reliable.
  70. The QSC has good psychometric properties for Children aged 30
  71. to 60 month, but less consistent one under the age
  72. of two.
  73. A systematic review and the U.
  74. S. And Scandinavia also agreed that the QSC had limited
  75. efficacy in detecting social emotional delays in Children 18 to
  76. 24 months.
  77. Therefore, research supports the age interval should be screened with
  78. the A S qsc over the age of 30 months.
  79. Another key factor in the literature review is the comparison
  80. of other screening tools to the ask SC.
  81. A statistical significance was noted comparing the S Q three,
  82. which screens all developmental domains compared to the S Q
  83. S e.
  84. Regarding personal and social scores, the lack of consistent screening
  85. in the primary practice setting for social emotional delays.
  86. A significant aspect is noted in the intervention group that
  87. receives the S QSC reporting increased knowledge regarding communication skills,
  88. response to cues and ability to soothe and establish a
  89. relationship with a child.
  90. A vital concept for the sqs e is providing education
  91. to patient and families which will focus on the child
  92. parent interaction.
  93. Providing tools and resource is within the local community by
  94. using key terminology, including young Children.
  95. A sqs key developmental delay and developmental screening enabled the
  96. literature search process to be streamlined and fruitful, which provide
  97. the best evidence based research to support the clinical practice
  98. by exploring the diagnostic screening tool specific social and emotional
  99. development research supports that the sqs E is more than
  100. a screening tool, but also an educational resource that can
  101. be used by providers at healthcare visits to increase knowledge
  102. regarding social emotional development and early childhood.
  103. A key factor that came up consistently in most of
  104. the studies was reliability, sensitivity and specificity of the sq
  105. s E was less consistent in translated adapted versions in
  106. Children under the age of 24 months.
  107. Future research focuses on translating the QSC into different languages
  108. on a global scale, even though the locations of the
  109. research studies varied from health care settings to early childhood
  110. education, setting results were consistent of the supporting variables, including
  111. the age being over 2.5 years, and the A s
  112. qsc increasing identification of social and emotional developmental delays.
  113. Compared to the S Q three or no screening tool
  114. being implemented, the S qsc can be completed independently by
  115. the caregiver or with the assistance of a health educator
  116. and reviewed with the health care provider at the well
  117. visit to make a diagnosis and offer and refer, early
  118. intervention service is Theis.
  119. QSC should be administered at the 30 36 48 or
  120. 60 month well visit and can be reviewed with the
  121. health care provider at the appointment.
  122. Information obtained from the S QSC can be used to
  123. diagnose social emotional developmental deficits and also identify patients for
  124. early intervention.
  125. Service is that can assist educational providers.
  126. That helped me Grow Committee is a group of professionals
  127. and San Luis Obispo County that is involved in an
  128. area of either early education.
  129. Early intervention service's healthcare and these areas would all benefit
  130. from early identification of social and emotional developmental delays in
  131. Children within our county.
  132. So how do we implement the QSC into the clinical
  133. practice? Integrating the sqs E into the practice setting is
  134. quick because the caregiver can complete the survey before the
  135. appointment and the review.
  136. The results with the provider incorporating the sq into visits
  137. is feasible and can be billed directly to Medicaid or
  138. private insurance company under the I C D 10 code
  139. z 13.4 encounter for screening of certain developmental disorders and
  140. build for $59.90.
  141. This diagram shows two ways that we can implement the
  142. sqs into clinical practice.
  143. The first child A is with the assistance of a
  144. health educator encounter.
  145. This means the caregiver and the child would come 30
  146. minutes prior to their well check with the provider.
  147. Complete the ESC VSC.
  148. Therefore, the health educator would be there for the parents
  149. to ask any questions.
  150. The results would be given to the provider, and then
  151. they would have the appointment and go over any questions
  152. or concerns and a diagnosis and support for referral would
  153. be implemented.
  154. Child B is also the provider establishes contact with the
  155. caregiver prior to the well check.
  156. This would be done either male or email through the
  157. Brooks platform.
  158. Once the caregiver completes the sqs be at home.
  159. The results will then be shared with the provider, either
  160. electronically or via mail.
  161. At the appointment, the provider would discuss the results and
  162. with the caregiver and make any referrals.
  163. If indicated.
  164. As a result, the child would have increased access to
  165. mental health Service's and support within sailor Service.
  166. Polk County?
  167. Yes. So how would we evaluate the implementation of the
  168. sq in the clinical setting evaluation of the plan should
  169. in crude reviewing the data every month?
  170. The metrics for the sqs see that needs to be
  171. evaluated is the number of screenings the sqs E scores,
  172. referrals, caregiver and family feedback surveys.
  173. Referrals for early intervention service is follow up visits that
  174. need to be done within a 2 to 3 months
  175. time period to see if the sqs score remains elevated
  176. even after intervention.
  177. Service is provider participation.
  178. Feedback Service's identifying and diagnosing Children with social emotional delays
  179. through the sqs E and providing early intervention service's is
  180. imperative because it will reduce the need for long term
  181. management of mental health conditions.
  182. Failing to detect social and emotional deficits early on increases
  183. the financial burden and can lead to an individual with
  184. long term mental health condition that's extend into adulthood.
  185. Okay, the scope of the project is that with the
  186. support of the San Luis Obispo helped me grow committee
  187. and all of its members implementing routine and systematic surveillance
  188. and screening specifically for social emotional developmental delays with the
  189. sqs e at 32 16 month well visits can impact
  190. the delivery of health care on a local and state
  191. level. First five of California facilitated the establishment of the
  192. Health Negro Committee, which is continually exploring areas of improving
  193. health and wellness for Children.
  194. And their development, however, only recently expanded into the health
  195. care initiatives regarding social and emotional mental health.
  196. The S qsc can be utilized by both private primary
  197. care and government funded pediatric clinics in San Luis Obispo
  198. County. Identifying and providing early intervention service is in childhood
  199. is imperative to reduce long term mental health conditions.
  200. Aligning with help me grow committees core principles.
  201. I have attached the list of references for you to
  202. review any questions or concerns.
  203. Please call me back.
  204. I look forward to hearing any feedback, Thank you very
  205. much.
