  5. and Eric an improvement oven area of health For the
  6. scenario, a patient has been having consistent high blood sugar
  7. and is a type one diabetic, not maintaining a suitable
  8. lifestyle to start off.
  9. The question would be, Is it okay if we discussed
  10. your blood sugar levels?
  11. The next step would be determining the patient's readiness.
  12. Um, when determine the patient's motivation, it's crucial to determine
  13. it based on the readiness to change ruler.
  14. When the patient shows disinterest, consider exploring how their condition

  16. In order to help identify the benefit benefits of a
  17. lifestyle change scenario, a Type one diabetic patient has addressed
  18. a concern regarding his or her high blood sugar level.
  19. Based on your current state, how likely would you be
  20. willing to make a change if the patient agrees to
  21. make a change?
  22. It is important to discuss after whether or not the
  23. patient has already started progressing towards a new kind of
  24. goal, whether that be a change in their lifestyle.
  25. Eating habits, physical activity.
  26. Any of those, um, and then if the patient has

  28. make a change.
  29. Identifying some potential barriers, such as kidney damage or potential
  30. blindness. Cardiovascular disease.
  31. All of those, um, bad things that come along with
  32. diabetes and risk factors will help the patient, um, be
  33. more motivated to make a change.
  34. The next step would be starting, UM, creating goals for
  35. the patient.
  36. Once a patient has identified the primary issue and has
  37. begun the process of starting a change, it's important to
  38. identify a goal.
  39. UM, goals can be measured and change and adjust.
  40. It is necessary, but it's important to track the patient's
  41. progress. It's also important to allow the client to voice
  42. what they feels feasible goal for them that whether or
  43. not in over their head or becoming stressed and don't
  44. progress towards their goal.
  45. The scenario for this one is the patient has identified
  46. the main issue and has decided that in order to

  48. lifestyle change, and they've chosen to begin eating healthier.
  49. The patient wants to reduce the amount of food eaten
  50. eaten that has high amounts of sodium and saturated fat.
  51. The three dietary goals that they've set are to stop
  52. eating sour cream and use plain Greek yogurt as a
  53. substitute. To stop eating ground beef onto begin eating ground
  54. turkey instead and to use plant based oils instead of
  55. margarine and butter after the patient has determine their goals,
  56. Breaking them down into a feasible attainable goals is the
  57. next step, Um, by breaking these down into smaller parts
  58. that will allow the patient to move forward without feeling
  59. feeling overwhelmed.
  60. Um, the scenario for this is that the diabetic patient
  61. has chosen to substitute a few foods they consume frequently
  62. that may have a negative impact on their health.
  63. They've set a weekly goal to substitute at least five
  64. meals made with a healthier ingredients selected to be replaced.
  65. Um, they want to work up each week, eventually eliminating
  66. the three foods from their diet completely after determining the
  67. goals, um, and breaking them down, setting up time constraints
  68. and measurements and just creating good an all around good
  69. plan filling out an action plan for my actually solidifies
  70. them. It allows the patient to be able to look
  71. back in reference what was discussed during the period of
  72. time see what their goals are.
  73. See how they're progressing, as well as allowing the case
  74. manager determined the patient's progress forward.
  75. Yeah, there's just a couple key points to keep in
  76. mind when creating an action plan.
  77. It's important to keep the client schools in mind, ensuring
  78. that they're directed at what they're targeted outcome is for
  79. their health, making sure that the goals are attainable for
  80. the patient.
  81. Ensuring that the progression notes that are being taken by
  82. the case manager are up to date and showing positive
  83. progress in the right direction, as well as communication occurring
  84. frequently between the patient and the client.
  85. To make sure that the patient is happy with their
  86. current situation, that the patient doesn't feel overwhelmed or struggling
  87. and that the goal is still within.
  88. Like, um, what they're targeted outcome is for their health.
  89. Um, it's also important to make sure that they both
  90. have a good time frame there.
  91. Not too long, not too short, something achievable, something that's
  92. going to give the patient, um, gotten satisfaction when they've
  93. completed a part of their goal or when they've made
  94. progress towards, um, they're targeted health outcome
