We would like to help you have a successful OPT application
First check that you have all the items on the OPT checklist. The checklist can be found in the OPT packet (www.emich.edu/oiss/forms.html).
Make sure your documents are in order on the checklist- this is helpful when you drop your packet off!
Check that all the forms are complete!
Remove all staples- this is requested by USCIS as they scan OPT requests
Make sure you are submitting your most recent F-1 visa
Academic Advisor Letter, Initial I-20, Transfer Pending I-20s
Make sure the information on your academic advisor letter is correct.
Be sure to include your initial I-20. It will say "Initial Attendance"
Transfer I-20s will say "Transfer Pending from (previous school)"
I-20s that state "Continued Attendance" are not needed.
The instruction page of the I-20 is not needed.
Passport Biographical Page
The page in your passport with your picture and expiration date of your passport
Make sure your passport is valid 6 months into the future!
Remember to include start and end dates according to the table.
Also, Initial OPT is only for 12 months. The end date should be one year later and the day before the start date.

Make sure it is typed!
the photos are the correct size, taken within 30 days, there are no shadows and the background is white.
you are not wearing eyeglasses
your name and SEVIS number are lightly written on the back.
Make sure the ink is dry before putting the photos on top of one another. You do not want ink on the front of your photo!
Include your complete SEVIS number (including the N) on the memo line
Review the OPT packet to make sure all the sections are correctly filled out.
Make sure the I-765 is typed!

Submit Initial Attendance and Transfer Pending I-20s.
Continued Attendance I-20s are not needed.

If you came to the U.S. by air, you were processed electronically. You can print your I-94 from https://i94.cbp.dhs.gov/I94
A copy of your travel history is not needed.
Do not staple documents- USCIS scans all applications and staples delays the processing
Possible Other Documents
Front and back of your EAD card(s) and I-20s requesting OPT (the instruction page is not needed).
If you have a job offer that you want to include this on your OPT I-20, please include offer letter and
3-5 sentences explaining how the job relates to your degree program.
**Without the offer letter and explanation, SEVIS will not allow the job to be added to the OPT request.
