Healthy PARTNERSHIP for Adolescent Obesity

The Goal
The purpose of Healthy Partnership for Adolescent Obesity (HPAO) is to work with 100 children regarding the issue of obesity. This is a community-based program where health care practitioners partner with schools and the community to offer education and strategies to improve the issue of obesity.
Adolescents 13-17
Key Activities
Spread the word and gain support for HPAO
Develop online modules for client use
Develop brochures and training modules for community support
Collaborate with schools regarding their health/physical activity curriculum
Train with use of Map My Run application
Revenue Stream
$150,000 from the Carol M. White Physical Education Program (PEP) grant
Quantitative Data
Survey feedback from stakeholders (children, parents, schools, health care providers).
Reports from modules website (hours of use)
Reports from students' grades (school physical education curriculum)
Map My Run reports (time spent on exercise).
Before, during, after weight, BMI, and measurement data.
Monitoring of budget (use of Excel spreadsheet).
The Problem
20% of children are predicted to be obese by 2050 (CDC, 2014).
The obesity rate of children aged 12 to 19 increased from 5% to 21% over a 32 year period (CDC, 2014).
There is a lack of exercise regiments for adolescents; adolescents live sedentary lifestyle (Hills, Anderson, & Byrne, 2011).
The Mission
To develop a partnership between the health care community and community parents to empower parents and their children to achieve healthier lifestyles, through increased education, improved clinical care, and a focus on behavioral changes.
