DepEd Orer No. 73, S. 2012:
Assessment shall be used primarily as a quality assurance tool to track students’ progress in the attainment of standards, promote self-reflection and personal accountability and for one’s learning, and provide a basis for the profiling of student performance. It should be holistic, with emphasis on the formative or developmental purpose of the quality assuring students’ learning.
"The attainment of learning outcomes as defined in the
standards shall be the basis for the quality assurance
of learning using FORMATIVE ASSESSMENTS. They shall
also be the focus of the SUMMATIVE ASSESSMENTS and
shall be the basis for grading at the end of instruction."

A widespread, common misconception of the quality of a test is that a good test should be a challenging test. "Challenging" is typically taken to mean difficult. Interestingly, given the goal of discrimination, a very "challenging" test is actually counterproductive. Imagine a test that is very difficult such that almost all examinees score near zero and their scores are very similar to that of everyone else.
Reliability refers to the precision of the test scores. The converse of the concept of reliability is the concept of measurement error.
It should be justifiable and effective. It is also the degree to which evidence supports the score-based interpretation and decisions (American Educational Research Association, 1999)
It should have a clear instruction on how students are going to answer the examination.
TEST BANKING is the technique of pre-depositing your
written tests, summative tests and even formative tests
into storages that are segregated according to their
level of assessment such as KNOWLEDGE, PROCESS/SKILLS,
UNDERSTANDING, and PRODUCTS/PERFORMANCE.
Teachers will have the opportunity to select the possible questions that they could include in making a test. It also provides organization of the test construction and it ensures the validity of the questions to be given to the students.
