Things to do when writing a questionnaire.

1) Make sure there is a time frame if necessary.
2) Make sure options don't overlap.
3) Have enough options for a question.
4) Don't ask personal questions or questions
that are likely to offend.
5) Ensure answers are balanced.
6) Make questions simple enough to understand.
7) Avoid bias in the questions or answers.
