Person Centered planning is a way of enabling people to think about what they want now and in the future. It is about supporting people to plan their lives, work towards their goals and get the right support.


Purpose
To look at an individual in a different way.
To assist the focus person in gaining control over their own life.
To increase opportunities for participation in the community.
To recognize individual desires, interests, and dreams.
Through team effort, develop a plan to turn dreams into reality.
To enable people to live the lives they want in their communities.

Who are the important people in the person's life?
What are the person's strengths (or gifts)?
