Gastro-intestinal pain
Dispensary
Ms. Rowena, a 32-year-old woman, comes to the dispensary asking for a pharmacist conversation. She told you that she recently received a prescription of “Colpermin” from a General Physician.
She says that the Colpermin gave her terrible indigestion and she has already been taking Alu-Cap capsules (self-prescribed) ever since she felt uneasy with her stomach.
Later, since Colpermin gives her a terrible effect, she adheres to Alu-Cap and also decreased the number of Colpermin capsules she was taking per day (incompliance).
She feels that her problems are just getting worse: first she had constipation, then, stomach cramps and then, bloating. Now she has indigestion.
She wants to know if you can sell her any better medicine for her current situation. She is now hesitant to see the Physician for she knew that the doctor would still push to continue Colpermin.

Colpermin Capsules
The General Physician give Ms. Rowena Colpermin to treat her constipation, stomach cramps and bloating. Since this medicine helps relieve the symptoms of irritable bowel syndrome (IBS). It belongs to a group of medicines called antispasmodics. The main ingredient peppermint oil, works by relaxing muscles and relives muscle spasms of the bowel in irritable bowel syndrome (IBS).

Alu-Cap
Alu-Cap is recommended for use as a phosphate binding agent in the management or renal failure. it may also be used as an antacid. Alu-Cap can be used for the relief indigestion. It neutralises the acid in the stomach and helps to protect the stomach lining from acid irritation. Alu-Cap may also be used to reduce the amount of phosphate ions absorbed by the body from the diet.

The symptoms of Irritable Bowel Syndrome (IBS) can be managed by changing your diet, lifestyle, and understanding the nature of the condition.
Patients with IBS are advised to monitor symptoms, diet changes, increasing dietary fibers, exercise, and reduce stress.
A. What are the different kinds of Gastro-intestinal pain?
A. Visceral Pain - directly related to the organ involved. The majority of organs do not have an abundance of nerve fibers, so the patient might experience mild or less severe pain that is poorly localized.

B. Parietal Pain - occurs when there is an irritation of the peritoneal lining. The peritoneum has a higher number of sensitive nerve fibers, so the pain is generally more severe and easier to localize.

C. Referred Pain - A visceral pain that is felt in another area of the body and occurs when organs share a common nerve pathway. For this reason, it is poorly localized but generally constant in nature.

As a Pharmacist...
Alu-Cap may not be suitable for everyone and some people must never take it. As a pharmacist, it is best to tell her to stop taking the said medicine and go back to her physician.
