STATE: 1. Lack of thorough and serious review to the initial design.

What lead the disaster to occur?
Failure of the cross-beam at weld joint
Significant change of original design.
It required the whole of the rod below the fourth floor to be screw threaded.

These threads would probably have been damaged and unusable as the structure for the fourth floor
Why the design was changed?
Lack of proper communication between Jack D. G, Associates and Havens Steel.

The drawings prepared by Jack D. G and Associates were only preliminary sketches but were interpreted by Havens as finalized drawings.

Jack D. G and Associates failed to review the initial design thoroughly, and accepted Havens' proposed plan without performing basic calculations that would have revealed its serious intrinsic flaws

State: 1.The signification mistake occurred but not engineers noticed that.

The structural component redesign work should return to consultant.



State: 3. The redesign specification required approval from the Engineer.

The contractor Engineer should be able to realize the redesign specification is not qualified but still approve it.
The contractor Engineer should check the submitted specification carefully.
