Teacher becomes concerned with students performance/behavior
Teacher will begin intervention
Teacher monitors Student's progress towards goal. Was the intervention successful?
Continue Intervention and document progress
Develop and follow Classroom Intervention Plan
Progress Monitor the students progress towards goal.
Is the student making sufficient progress?
Teacher makes referral to SIT coordinator (SIT-1 Initial Intake Form)
SIT coordinator ensures all documentation is collected prior to SIT meeting
SIT Meeting: Team discusses individual students using all available data.
SIT suspects a disability
SIT recommends continued/additional interventions
Refer to 504 Coordinator
Begin special education referral process
SIT 2 - Notification of Referral
SIT 3T- Teacher Focus of Concern
SIT 3P - Developmental History
SIT 4 - Hearing & Vision Screening
Classroom Intervention Plan
Required Documentation:
Student Intervention Team Process
SIT 6 - Progress Report
If the student will continue in the SIT process, the SIT Coordinator sends home SIT 5 - Notification of Intervention.
