Physical Education Uniforms
Currently there are five middle school health & physical education teachers, they are as follows:
1. Kelley Melvin
2. Britney Ewan
3. Erin Lynch
4. Cory Forman
5. Scott Land
Each marking period, we will have four to five bi-weekly participation grades which will count for 85% of the students overall grade.

The remaining 15% of the physical education grade will be comprised of minor assessments that tests the students' knowledge on components of particular activities.

Usually, minor assessments will be given every three weeks following the completion of an activity block. Students will be provided review information of some kind prior to the assessment.
Each student in the middle school will have three marking periods of physical education and one marking period of health education during the school year.
Each student's final physical education grade will be the average of the three marking period grades. The health education grade is it's own subject and grade.
Each student will have their Health and Physical Education Class every other day and each class period lasts sixty-five minutes.

Change into uniform (five minutes),
Attendance/Warm-up (five to ten minutes),
Skill Work/New Movement Development (fifteen minutes),
Workout/Active Participation (ten to twenty minutes),
Skill Work/Challenge/Game (ten minutes),
Cool Down/Review Discussion (ten minutes),
Change out of uniform, dismissal (five minutes)
Every student is evaluated on a day-to-day basis. Points are deducted from a student's grade when their behavior and/or performance is ruled less than satisfactory and does not contribute positively to class.
Each day a student has physical education class will be worth twenty points toward their bi-weekly grade.
Students will receive ten points for being prepared for class for that day.
Then, students will earn up to ten points based on their level of participation. (For example, A student that is highly active, self-motivated and models positive behavior would earn the maximum ten points available for that particular day.)

Students must wear the approved Clearview Physical Education Uniform. The uniform consists of green shorts, a yellow t-shirt and sneakers of their choice.

Students are to put their last name on both their shirt and shorts.

Students are allowed to wear warmer clothing, that is not green and yellow over their uniform, when needed.

Sharing uniforms is not permitted.

Clearview Middle School Health & Physical Education
Students are not permitted to wear jewelry of any kind during activities.
However, new piercings can be worn up to eight weeks with a note from the piercing establishment.
A newly pierced ear lobe can be worn, if taped for six weeks. Newly pierced cartilage can be worn, if taped for eight weeks. It is recommended that students tape their new piercings with athletic tape.

Each student is permitted two medical excuse notes from a parent and/or guardian each marking period.

Parent/Guardian excuse notes must be written.

Parental/Guardian phone calls during the school day will not be honored.

Each note will excuse the student for one day only.

All notes, including notes from a physician, must be taken to school nurse before first period.
Students do not have points deducted from their grade when their absent.

However, students are responsible to make-up any assessment that they may miss due to an absence.

Safety and Proper behavior are stressed at all times.

All class injuries must be reported to the instructor immediately.

It is recommended that students lock up all of their personal items with a combination lock during physical education class.

Lastly, no aerosol spray, colognes and perfumes are allowed in the locker rooms.
A typical physical education class will generally look as follows;

Our vision is to increase the student's general physical preparedness and physical competence in the ten fitness domains; cardiovascular endurance, stamina, strength, flexibility, power, speed, coordination, agility, balance and accuracy. Ultimately, preparing the student with the skill, technique and confidence necessary to pursue any physical challenge that they desire.
Mission Statement
The mission of physical education is to have students regularly participate in activities that incorporate functional movements that require students to pull, push, run, throw, climb, lift and jump.

He is beginning his fourteenth year at Clearview.
He has earned a Master's Degree in Health & Exercise Promotion from The California University of Pennsylvania
He has earned a CrossFit Level One Certification
He is married with two children, Chase, 5 and Avery 3.
Facts About Mr. Forman
He is beginning his thirteenth year at Clearview
He earned a bachelor's degree in sports administration/exercise science from The Ohio State University.
He earned a master's degree in health and physical education from Emporia State University in Emporia, Kansas.
He is married with one Child.
The Health and Physical Education Staff encourages all of our students to believe in their abilities, set goals and put forth their best effort each and everyday. We look forward to the 2014 -2015 school year.

She graduated from Rutgers University.
She coaches soccer & gymnastics.
She likes to run, train in gymnastics and play soccer.
She is beginning her ninth year at Clearview.
She is the Clearview Field Hockey Varsity Head Coach.
She is the Middle School's Peer Mediation Advisor.
