Dental Prophylaxis--This is for patients who exhibit healthy gingiva and it includes a caries exam, OCS, probing, scaling and polishing to remove plaque, calculus and stains from the tooth structures in the permanent and transitional dentition.

Periodontal Maintenance--Following periodontal treatment (surgical and/or non-surgical) and once the patient is stable, this appointment includes a caries exam, OCS, probing, removal of bacterial flora from crevicular and residual pocket areas, scaling and polishing of the teeth, periodontal evaluation and review of patient's plaque control.
Scripting for transitioning patients from a 6 month maintenance to SRP then a 3 month maintenance is important!
We have been noticing some changes in your gum tissue and are very concerned. We need to take a step back and non-surgically treat your gums to avoid infection and future tooth loss.
Suggested wording may be....
Since your last visit, we have noticed an increase in bleeding and an increase in the depth of your periodontal pockets. Current research shows that non-surgical intervention may help you to avoid surgery and tooth loss in the future.
