What is Disease Registry ?
Generational cohort (demographics), an aggregation of individuals who experience the same event within the same time interval.
Cohort (statistics), a group of subjects with a common defining characteristic—typically age group.
To create a Cohort Builder based on certain conditions, and creating an application for the same using MEAN stack.

Typically, the complex questions of the clinician experts must be transmitted to technical team members, who then conduct the queries and produce a custom-built report one that will likely require several iterative reviews before it meets the needs of the clinicians. This process of iteratively defining patient cohorts can be time-consuming and it delays progress on meaningful clinical improvement and research.

A disease registry is a special database that contains information about people diagnosed with a specific type of disease. Most disease registries are either hospital based or population based.
A hospital-based registry contains data on all the patients with a specific type of disease diagnosed and treated at that hospital.
A population-based registry contains records for people diagnosed with a specific type of disease who reside within a defined geographic region

The system is presently reactive, but there is a future scope of for making cohort explorer which will use proactive approach that will make the system identify the trend.
Example: The system will automatically suggest to make different medical programs on the basis of histograms, pie-charts, or bar graph.
