Coordinated Intake in
Southern Nevada

Self
Homeless client presents at a hub, on his or her own, for housing assistance.

Provider
Homeless individual presents at a homeless service providing agency (CoC) requesting assistance and gets referred to hub.

Homeless individual receiving services at an agency providing other services (not homeless services/non-CoC) gets referred to hub.

MCIT
MCIT encounters homeless individuals in the field and are able to route appropriate clients through coordinated intake.

Community Queue
The community queue is not a place but rather an electronic prioritization list.

Clients are prioritized in the queue by score and date assessed and are required to check-in 1x/month regarding their status.

Individuals from the queue are matched to vacancies using score on the community housing assessment, specific client information/responses, and program eligibility.

When matchers have matched an open bed with an eligible client, then the matchers ‘refer’ the client to the program in HMIS.

Clark County Social Service
Client must be able to make it to a hub for intake/assessment.

Transportation is not provided by the hubs or MCIT for provider referrals.

Engagement
MCIT teams are deployed daily.

Can provide the required monthly check-in for clients encountered in the field who are already in the community queue.

If appropriate, MCIT can complete a community housing assessment in the field OR can transport client to a hub for assessment, depending on client preference and comfort.

Walk in through security at the front.

Client is given a number and is greeted by a public service specialist (PSS) at the front desk when client’s number is called.

Client will complete an application at a kiosk (PSS will be available to assist if needed).

Whenever possible, clients will be assessed same day. However, generally, clients will be screened for benefit eligibility and scheduled for a return date.

Go through security

Client will be called by the eligibility worker or social worker by name.

Eligibility worker brings client back to his/her office area for the interviewing/assessment.

Eligibility worker will conduct the appropriate assessment(s), including the community housing assessment.

Community
Housing Assessment
The assessment
objectively
determines what type of housing is most appropriate for the client.

Not all clients referred to the hub for housing are eligible for Transitional Housing, Permanent Supportive Housing, or Rapid Rehousing.

Once the community housing assessment is completed, if scoring indicates eligibility, client will be referred to the community queue in HMIS.

Field Community
Housing Assessment
The field community housing assessment is the exact same objective assessment as the one conducted at the hubs and will provide eligible clients with the same housing opportunities.
