Health Promotion Plan

Diabetes support group provided by Humboldt General Hospital first Wednesday of every month.

Nutritional assessments completed by Humboldt General Hospital dietician.

Nevada Diabetes Association holds picnics, camps, and information sessions for diabetic patients

1. Diabetes support group in high Native American populated area, such as McDermitt.

  • This is my first objective because, in order for Native Americans to come to a support group in Winnemucca they have to drive 75 miles. Having a support group in Mcdermitt will allow more individuals to attend the class.
  • I can measure my objective by keeps attendance of the group and monitoring which individuals come each time.

Native American Relation to Diabetes

Health Promotion Plan

According to the CDC, Native Americans are two times more likely to have diabetes than Caucasians.

Native Americans are the leading ethnic group for diabetes.

Native Americans are more prone to diabetes due to

  • Sedentary life styles
  • Diet high in fats and carbohydrates
  • They are more insulin resistant than other ethnic groups

2. Have average blood sugar levels daily and have decreased blood sugar levels in support group members.

  • This objective will get members into the habit of checking their blood sugar everyday.
  • It will make members aware of blood sugars within normal limits.
  • This can me measured by members writing down their blood sugar everyday, and monitor it at the meetings.

  • Helping members attain glucometers to monitor blood sugar
  • Assisting members in finding rides to primary care providers
