Frenchay Dysarthria Assessment - Second Edition (FDA-2)

What is Dysarthria?

  • According to ASHA, "Dysarthria is a motor speech disorder. It results from impaired movement of the muscles used for speech production, including the lips, tongue, vocal folds, and/or diaphragm. The type and severity of dysarthria depend on which area of the nervous system is affected" (ASHA, 2016).

Test Administration

  • Specific instructions included for each section of the assessment
  • Only score the client's second attempt for each task

Scoring

  • Each item is rated on a scale from "a" to "e," where "a" means normal for age, and "e" means unable to undertake task/movement/sound.
  • Use Rating Form provided.
  • Since I'm not an expert at assessing dysarthria, I was not sure if everything I was looking at was normal or not. For instance, my client's tongue was involuntarily moving during the tongue at rest subtest, and I was not sure if that would be scored as "a" or "b."
  • Issue: I think it may be hard to assess a client in the intelligibility subsection due to their accent, their cultural background, and whether or not the clinician has met and/or is familiar with the client's speech.
  • As with all assessments, a practicing clinician needs to make sure they are familiar and comfortable with the FDA-2 in order to have a smooth assessment.

Comments

  • No basal or ceiling; the examiner must perform all sections and each individual item within the assessment.
  • Validity: Face & Content validity; over 90% of the cases tested were predicted to belong in their correct diagnostic category.
  • Reliability:
  • Interrater scores were substantially or moderately reliable. However, there were no coefficients given.
  • Intrarater scores were all high (0.72-0.76), with one having almost perfect reliability (0.92).
  • Mean Standard Score & Standard Deviation: No one mean standard score or standard deviation; the FDA-2 provides these scores for each specific type of dysarthria the authors looked at in their study.

Psychometric Considerations

  • Interpretation: The manual provides charts that summarize the results of 5 different types of dysarthria
  • Upper Motor Neuron Lesions
  • Mixed Upper & Lower Motor Neuron Lesions
  • Extrapyramidal Lesions
  • Cerebellar Lesions
  • Lower Motor Neuron Lesions
  • Not culturally biased.
  • The examiner must gather a tongue depressor, stopwatch, tape recorder, glass of water, cookie (~ 4 inches in diameter), sterile gloves, and calipers on their own for the subtests/influencing factors.
  • Includes an examiner's manual, rating form, and intelligibility cards.
  • Appropriate Ages: 12 years to adults

STIMULUS MATERIALS

  • Unique: Unlike other assessments, a clinician plots the data on the Rating Form, and based on what the form looks like, it will give the clinician an idea of what type of dysarthria the client has.
  • The test was normed on a total of 194 normal, healthy participants ranging in age from 15-97 years.

Additional Information

  • Includes a section on influencing factors.
  • Divided into 7 sections: reflexes, respiration, lips, palate, laryngeal, tongue, and intelligiblity; each containing several individual items.
  • It is a rating scale with which clinicians assess patients' performance on a range of behaviors related to speech function.
