The fire accident happened because the tank contained explosive vapor from waste chemical and oil materials from truck service stations. A spark from welding ignited the fire.
The fire started at 10:00 a.m., on May 4th, 2014. Employees Ali Al-Ahmad and Ahmad Al-Ahmad, welders–Badge #: 98654 and 98612 were welding brackets onto an oil storage tank (55,000 gallons). The tank, half full, contained explosive vapor from waste chemical and oil materials from truck services stations. A spark from welding ignited the fire; the tank exploded; and the top was blown off. Heat/fire resistant clothing was not used. Hollow structures such as oil tanks were not tested before welding.
