Current Methods

What are the Current Methods?

Helsinki Method

Europe

The Helsinki Method beings with first receiving consent from the patients. They begin a cross testing of the donor and patient antibodies (HLA-typing) to make sure that they are a match. An immunosuppressant regimen is set up as well as rejection monitoring. The patient is then screened for infectious diseases.

"The entire surgical procedure (tissue harvest and implantation)

took a total of 24 hours and there were no major complications dur-

ing surgery. The patient required a total transfusion of 20U packed

red cells. Good pulsatile arterial bleeding was observed throughout

implantation of the donor tissue with no evidence of ischemia. The

patient was fully awake and receiving spontaneous ventilation within

24 hours after surgery." (Lippincott Williams & Wilkins)

In Spain a face transpalnt was performed on a 30-year-old white male who suffered an accidental gunshot to the face. All scar tissue and bone fragments were removed from the recipient’s face. They Implanted the facial allograft by reattaching the blood supply. Arterial revascularization was achieved with an end-to-end anastomosis of the right external carotid artery.

Ethical and Technical Problems

CURRENT ISSUES

- Technical

- Transplant rejection: Patients would need to take drugs to suppress the immune system, but these drugs have harmful side effects!

- Thrombosis of arteries and veins: rarely happens during free flap surgery, but with larger areas of tissue being transplanted, there's a greater chance of this happening.

- Ethics:

- How would you feel if your family member's face was on someone else's body?

Focus

Goals

GOAL

  • Customized Face Transplant
  • Sustainable Transplants
  • Reduction of Depersonalization Disorder
  • Vasculature/Angiogenesis
  • Provides Transport to All Interfaces
  • Need to Increase Transport Capabilities

Interfaces

  • Vasculature/Angiogenesis

  • Muscle, Fat, and Skin Tissue

  • Nerve Connections

Interfaces

Significance

Importance

- Improving quality of life:

- Eliminating wait-time on donor list

- Many recipients are survivors of suicide attempts

- Improved healing process:

-Physical: better interface connection with original tissue since it is made from your own stem cells

-Emotional: feeling like it is your face, reducing risk of depersonalization disorder

FUTURE

- Creating 'designer faces'

- new plastic surgery trend

- anti-aging for ~living forever~
