CHAPTER 9 – REPORTING AND ANALYSIS OF NON - COMPLIENCES, INCIDENTS/ACCIDENTS, HAZARDOUS EVENTS, COMPLAINTS and SUGGESTIONS
A near miss is an unplanned event (or sequence of events) that does not result in injury, loss or damage but has a high potential to do so. Only a fortunate break in the chain of events prevented an injury, fatality or damage. Near misses are more frequently encountered every day and at any stage of shipboard operations.
Description of the Event
* Benzene discharging operation has been completed in Huelva,afterward shore informed that the vessel could disconnect the arm,accordingly pumpman started to wear the SCBA set and light chemical suit (PE) properly.
*A/B on watch saw a shore staff without PPE who was trying to help ship’s personnel for disconnection, afterward A/B & Pumpman warned him immediately and kept him away from the manifold.When disconnection commenced,there was a splash and leakage due to insufficient drain and depressurizing of the shore line/arm.
* Personnel in charge at the manifold was wearing the suitable protective equipments and luckly they have warned the shore staff who was w/o PPE on time,so there was no injury.

A hazardous occurrence is a condition in the environment, equipment, article, substance, machine, installation or situation which does not cause to an accident or a near miss but has a high potential to do so as long as the situation or environment in exists as is.
*During monthly check of portable fire extinguishers, there was one CO2 tube was located in front of the refrigerator cooling fan in galley, so that it’s body and foundation were heated due to hot air flow from fan, the safe working scale of the mentioned extinguisher is between -20 deg. and +60 deg., afterward we have checked the place of the minimax as per fire plan and it was found that place was wrong, so we have changed its position as per Fire Plan.
*If this situation were not realized, heated extinguisher may explode, and it may cause serious personal injury. Or the cylinder may not be operational due to environmental effects and in any hazardous occurrence vessel can not use subject extinguisher for firefighting.

* If, we could define the hazards as per explained concepts. It would be easier to report.
* Even the vessel would not be facing with near miss occasions, it is for sure there will be always an hazardous occurrence to report.
