These visits will continue with the given excersises with updated resistances/reps to allow for proper muscle strenghtening/stretching based off of the communication between the PT and PTA from the last visit. The communication also includes for how Gina reports her left knee pain over the course of the week (assuming the visits have been made for once a week to allow for a better time frame of care given). Any questions or reviewing can be done with the PT or PTA if both are on site. Plan of care will be continued and adjusted as needed.
After communicating with the PT about the new patients guidelines, expected goals, and currated flow sheet to follow the PTA will have a better overall understanding of what both the PT and patient are hopeful/expectant of throughout this plan of care. Most might think that since Gina is a marathon runner she could handle anything that would be asked of her in terms of excersises, but it is ultimatley up to the PT to both limit and increase the difficulty of the excersises at the begining of the visits. So the PTA will follow the excersises with Gina and take note of how she holds up with the amount of reps she can produce with each excersise, which movements she has more/less difficulty with and any questions she might have about her home excersise program. At the end of her visit the PTA will implement the type of modalities (if applicable it could be at the beginning) that the PT has reqested for Gina. The PTA will communicate through EMR/face to face with the PT to review Ginas plan and see if any progressions need to be made for the next visit.
Possible discharge day. Overall the PT will review the patients progress and have an indepth disscussion of their progress or lack of and decide the next course of action. The last approved day of visits does not always mean that you are discharged. If nothing has resolved itself from Ginas left knee pain then it might be time for some scans of her knee before asking her doctor for more prescribed visits. If everything has resolved itself and Gina along with her doctor are happy with her results of growth then she can be discharged with an upgraded Home excersise program. The program can be drawn up and revised by the PT or PTA as appropriate. Gina can always request a new referal for any pain that may arise in the furture and start a new care treatment with the clinic if need be. More often than not bands can/will be sent home with the patient so as to continue their home excersies program if they do not have any access to gym equipment. The discharge will be sent to the physician by the Physical Therapist.
This visit is considered the half way mark of the plan of care so having updated measurements compared to that in the Evaluation Day are taken to report to the doctor Gina was reffered from for her knee pain. Has she noticed any improvements herself outside of the clinic? Any new pains? Any lack of mobility that has/has not improved? The measurements can be performed by the PTA but the notation will be given by the PT to the physician.
On the day of the evalutation the tech will take the patients vitals, such as blood pressure/height/weight, and once done will notify the Physical Therapist that the patient is ready for them. The Physical Therapist will then gather information such as, pain levels, time frames of the incident, any past/current medical history, and what their goals are for being in physical therapy. They will take measurements of strength, range of motion, and current pain levels. With the gathered information on Ms. Gina, the Physical Therapist will create a Plan of Care best suited for her knee at her current state. They will go over what they should be looking for in further aggravated symptoms and what to work on at home/gym setting , if applicable, until the next appointment.
