45 Day Screening Process

Vista College Prep - Sueno Park

Why?

Why must we complete a 45 Day Screening?

Early detection = Earlier Prevention

  • Part of the federally mandated Child Find process to "locate, identify, and evaluate children with disabilities. "

  • 45 Day Screening is Arizona's answer to oblige with Title 34, Section 300.111 of the Individuals with Disabilities Education Act (IDEA).

This is not a referral for a special education evaluation, and this does not lead to one. This serves as a preliminary step to potentially establish tiered interventions.

Who?

Who Completes a 45 Day Screening?

  • Homeroom teachers and Core teachers will complete a 45 Day screening form for every new student to the Vista College Prep.

  • Students with IEPs do not need to be screened. Please turn in the student's 45 Day screening form (blank) if you happen to receive one by mistake. The form will be marked "IEP received/reviewed." Students with 504 plans do need to be screened if new to VCP.

When?

When must a student be screened?

  • Per state guidelines, a new student must be screened within 45 calendar days of enrollment.
  • You will receive 45 Day screening forms on Tuesday, September 6th with a list of students that need to be screened.
  • You will have time on Tuesday, September 6th to complete the forms.
  • All screening forms must be completed by EOD (4pm) on Friday, September 9th.

What?

What do we complete?

  • Each 45 Day screening form has 7 sections (vision, hearing, communication, cognitive or academic, adaptive, social or behavioral, and motor). Complete each section by checking YES or NO. If you have not observed the behavior or skill, check NO.

1. Use only black or blue ink

2. Put an X in each box. Do NOT put a line down a row.

3. If you make a mistake, we will reprint a new sheet. Do not cross items out or use whiteout.

4. Each form is meant to be completed individually (your perspective).

YES forms-parent information letters will be sent indicating a follow up action. Homeroom teachers will receive a scanned copy of letter(s) that go home to parents so that they are aware of what the follow up action will be.

  • Age and Gender
  • Data Collection
  • Notes to clarify - Be specific

Where?

Where do we return the screening forms?

  • Please return all 45 Day screening forms with anything marked YES to Ms. Turby. She will have a designated folder in her office for these forms.

  • Please return all 45 Day screening forms with all areas marked NO to Ms. Turby in her office.

  • All forms will be filled out on Data Day September 6th.
