Inotrope use in neonates

Neonatal Hypotension

Neonatal Hypotension

Improve tissue and end organ perfusion

Aim of management

Increase cardiac output/blood flow

Maintain vasomotor tone of local vascular beds

Increase oxygen and nutrient delivery

Maintain steady cerebral blood flow

Pressure driven approach

Is a pressure driven approach correct?

BP =

Cardiac output (CO)

x systemic vascular resistance (SVR)

Target MBP = gestation (little evidence to support when recommended by BAPM in 1992)

Targeting a number does not tell you whether you need to address CO or SVR

No consideration for underlying pathophysiology

No consideration for compensatory mechanisms, cardiac function or fluid status

CO = stroke volume x heart rate

SVR determined largely by smooth muscle cells lining the arterioles

Autonomic nervous system has a pivotal role via adrenergic receptors located on cell membranes of smooth muscles in the vasculature and myocardial cells
