Medication

MARS Using Kipu Meds 2.0

Overview

KIPU MEDS 2.0

  • The all-new Medications 2.0 has been specifically designed to save more time, eliminating virtually all roadblocks to 100% efficiency.
  • Medication can now be entered by the appropriate staff and approved from any smartphone, tablet, remote laptop/PC, by your physician.
  • Medication can be looked up against the FDA database screen for available dosages, potential contra-indications, and other factors.

Admissions Processes

Admission

  • Upon Admission all clients medication brought in need to be logged into KIPU.

  • This action will automatically generate a MAR sheet for the client.

Medication Brought In

  • Medications Brought In is the process to log and verify the medications the patient brought in on admissions in a special area under the Doctor’s Orders tab.

Log New Medication

  • In order to log the medication(s) the patient brought in under the

section

  • Click on button.

  • In the new pop-up window, add the details of the medication the client brought in.

  • NOTE: in addition to regularly required fields, Meds Brought In requires a user to add Medication Expiration Date and Amount Brought In.

  • Once ready, click on Submit.

  • After the medication is added, you can click on and icons to expand or collapse the medication details, notes, and attachments:

No Medication Brought In

  • If the patient brought no medications, you can also log this and have the client’s signature confirming this:

  • Click on button.

  • Confirm the action by accepting the popup “Please confirm the patient did not bring in any medication” warning. A new transaction will appear with a note No Medication Brought In by Patient.

  • If your Policies and Procedures require, you can add patient’s, witness’, and/or verification signatures.

If your policies and procedures require, logged and verified medications can be signed by the patient. You are able to add Patient’s signature at any time: before or after medications verification or witness signature.

To add the patient’s signature:

  • Open
  • Have the patient review all medications > click on button and let the client sign.

  • Once client signs, click on Submit to finish.

Medication Verification Process

  • After medications that the client brought in have been logged, the medical team decides what they do with each medication. Verification process allows to Store medication, Destroy it or Continue on Admission:

  • Store – If Patient Inventory feature is in use, Store action would keep medication in the patient’s inventory as inactive.

  • Destroy – Destroy action requires a Witness’ signature. If Patient Inventory feature is in use, this action would create a Dispose transaction.

  • Continue on Admission – allows creating a Doctor Order with this medication for the patient. If Patient Inventory feature is enabled, action would add a medication to inventory as active.
  • To verify the medication(s):

  • Open . The button will have [X] – a number of new medications available for verification.

  • Select a Status (Store, Destroy, Continue in Admission) for each medication you want to verify at this time, and a Witness if applicable (you can leave Witness drop-down empty).

  • Click Verify button.

  • If you have any Continue On Admission medications, the new window to create Doctor Order(s) will pop up.
  • You can modify start date and time, justification, warning, notes, remaining duration, frequency time and refills, as well as decide if you need to continue this medication on discharge.

  • Hit Submit

  • Once medication(s) verified, click on Verification Signature button to add the signature > sign and click on Submit to stamp the signature.

  • When a preexisting client is given a new medication it must be entered in as a new order

  • This can be accessed through the Doctor's Orders Tab

Creating New Order

  • To start creating new custom order(s), navigate to Doctor’s Orders tab in the chart. Click on button.
  • Once on the order creation screen, select Medication tab and start filling the medication’s details:

  • Medication, Route, Dosage Form – required fields. Select medication from the auto-complete drop-downs.
  • NOTE: You can add only one medication per order.

  • Start Date – adjust your order’s start date and time. The filed will populate the current date and time by default.

  • Justification, Warning – Justification field is where you enter the medication reason

  • Duration in Days – leave blank if it’s an open-ended order or add a number if it’s a consistent dose. NOTE: Do not type anything else in this field (e.g. days, the field accepts only integers).

  • Note – optional notes field.

  • Frequency, Strength (previously known as Dose), Units,

  • Quantity – How many tab or capsules per dose

  • Dispense Amount is calculated automatically based on your frequencies, doses and number of days (if applicable). You can also specify how many refills the order has.

  • Creating Interval Medication – if you are creating a standard medication with an interval frequency (e.g. every 4 hours, every 2 days), these are all fields you need to fill in. Make sure you selected appropriate Start Date and time, your interval will calculate from the order’s start time

  • Creating Scheduled Medication – if you are creating a standard medication with scheduled frequency (e.g. two times a day, at bedtime), you can also modify the time (At), Strength and Quantity for each dose you are planning to give to the patients.

  • Creating PRN – if you are creating a PRN medication (as needed), check off PRN checkbox. For Scheduled PRN medications, you can modify the time (At), Strength and Quantity for each dose.
  • If PRN medication is given a frequency with a set time (e.g three times a day, at 9pm), then it will show in the Scheduled Medications section with Type set to PRN

  • If a PRN medication is given an interval frequency (e.g. every 6 hours, every 2 hours), then it will show in the PRN Medications section

How to Schedule PRN

  • When entering a new order check "PRN"

  • After checking "PRN" enter frequency for the number of times a day the client should take medication.

  • It will be put in time intervals starting from when the client takes the first dose.

  • i.e if a client is to take PRN med every 6 hours and they start at 9am then the next dose will be given at 3pm.

Change In Order

  • Any active order can be changed after it has been created (including quick orders, custom orders, orders created from medications brought in by the patient, regular medication orders

Doctors Orders

  • Click on button next to the order you need to modify. In addition to usual order fields, Change Order screen includes order change history and a Med Log history. Once you made the necessary changes and click on Submit, it discontinues the current order and creates a new order.

Patient’s orders can be viewed in 2 modes:

  • Current Orders – contain all orders that are currently active or that have been discontinued.

  • All Orders – contain all orders ever prescribed to the patient during this episode of care, grouped by start dates.

Click on Show all orders / Show current orders buttons to switch between modes.

In both modes, orders can be sorted by Order name, Start/Stop date and Status. You can also use Search/Filter by keywords (applied to any visible text on this page, including doses, warning and doctor’s name).

How to Discontinue Orders

There are two ways how the user can discontinue the order

  • Multiple orders
  • Individual orders

Doctors Orders

You can easily identify Discontinued orders on the list view: orders have STOP: in the title and are red. On the individual order’s page, you will see Discontinued stamp:

  • On the main Doctor’s Order tab, click on

button

  • Pick all orders you need to discontinue > selected orders will be highlighted green

  • Select Ordered by and Via.

  • Hit Submit.
  • Click on the order you need to discontinue

Med Pass

Monitoring of Self-Administration of Medication

  • The Med Pass tab displays all patients that have medication due by the hour, for any of the previous 24 hours and the next 24 hours.

  • Click on any patient/client and you will be taken to their Med Log (also known as MARs or MORs).

Medication Administration Record

  • Once in the MARs all current medication that are due will be located at the bottom.
  • Medications section contains medical orders with scheduled frequencies and regular medical orders with interval frequencies . The type column helps to identify if medication is PRN or not.

  • Please click Yes or No to indicate whether medication was taken.

  • If the client did not take medication, please indicate the reason for refusal in comment section.

  • If the client missed medication and is taking medication later or if there are any special circumstances please note in comment box.

How to Observe/ Administer

When administering or observing the medication or action:

  • Find your dose/log on the list of Medications or Actions. Scheduled medications and actions can be sorted by each column as well as searched by keywords from the title, time, type or dose/strength – e.g. by Medication Name or PRN type.

  • Select Yes or No under Occurred/Taken column for scheduled orders > add a Comment if necessary > click on Save Status to complete the log.

  • For PRN medications and actions, pick the time, modify the Reason if necessary, and click on Administered By/Observed By to complete the log.
  • NOTE: once medication/action is logged, it cannot be changed or deleted (not even by a super admin.

  • click on the button next to patient’s picture and give client to sign.

Patient Inventory

  • On Inventory > Patient page, you are able to select the patient in your current census and see what medications the patient has in their inventory.

  • Each line represents a medication with a particular strength per dosage form, “pills”.

  • Each is color-coded:

  • Green- means the medication is active and in use,
  • Red – medication has expired,
  • white – the medication is not in use or empty.

Managing Medication

  • After the new medication is logged in the inventory, you can perform various actions with the pills. Every transaction requires a confirmation signature
  • Click on medication to open medication’s details > there click on

  • You will get the drop-down list of the following available transactions:
  • Spillage – to log pills spillage. Specify the Quantity you want to subtract from the Remaining amount.
  • Med Count – to perform medication count. Specify the current pills amount. If it’s different from the Remaining amount, the transaction will highlight yellow.
  • Lost – to log the pills which whereabouts you do not know. Specify the Quantity you want to subtract from the Remaining amount.
  • Returned – to log return of defective pack/pills to manufacture/pharmacy. Specify the Quantity you want to subtract from the Remaining amount (the entire remaining amount populates automatically).
  • Refill – to log the refill. Specify the Quantity you want to add to the Remaining amount (the original received amount populates automatically). Once the transaction is submitted, the number of remaining refills will decrease. Transaction required Expiration Date.
  • Disposed – to log pills you are about to destroy. Specify the Quantity you want to subtract from the Remaining amount (the entire remaining amount populates automatically). Transaction required Witness signature.
  • Add – to log more pills. Specify the Quantity you want to add to the Remaining amount. Transaction required Expiration Date.

How to Refill Medications In Inventory

  • Once the medication’s remaining amount reaches the Replenish alert level, or the medication expires, the system allows you to do an easy refill:

  • Click on

  • Medication, route, dose form and strength will be set, you only need to fill the rest of the fields the same way you do when adding the new medication.
  • Once ready, click on Sign and Submit.
  • That would create a new Refill transaction. A number of remaining refills will be reduced.
