Is there any patient prep and are there any precautions?

Virtual Endoscopy

Patient prep for a Virtual Endoscopy varies depending on where in the body the patient is getting the procedure done. Since there is an MRI the patient will be given a oral contrast solution to get a clearer view.

For a Virtual Colonoscopy, prep is done similar to a regular colonscopy. The patient must fast 24-48 hours before the procedure. The doctor will prescribe a oral laxitive solution to clean out the bowel and the patient should drink only clear liquids. Liquids should be discontinued 6-8 hours before the procedure takes place.

The patient should be cautious as this test should not be done if they are pregnant and/or have any metal in the body since the MRI is very sensitive to metal due to magnetic pulses put out.

What is a Virtual Endoscopy and what is its modality?

Virtual Endoscopies can detect a wide range of malignancies in the multiple different ways endoscopies can be performed.

An imaging technique in which cross-sectional images acquired by computed tomography or magnetic resonance imaging are processed by a computer. The computer will reconstruct a three-dimensional display similar to that seen through an endoscope of internal organs such as the bowel, colon, stomach, lungs and esophagus.

Virtual Gastrointestinal Endoscopy:

  • Gastric cancer
  • Gastritis
  • Stomach Ulcers
  • Erosions

Virtual Colonoscopy:

  • Colorectal cancer
  • Polyps
  • Ulcers
  • Lesions
  • Inflammatory Bowel Disease
  • Obstructed/or twisted bowel

Virtual Bronchoscopy:

  • Stenosis
  • Occlusion
  • Respiratory Papillomatosis (RRP)
  • Squamous cell carcinoma
  • Lung Cancer
  • Laryngeal Cancer
  • Bronchogenic carcinoma

What is unique about this test and how does it differ from other medical imaging techniques?

Virtual endoscopies utilize both CT or MRI imaging and computer algorithms in order to

recreate a real time 3D image of internal anatomy. This allows physicians to view tissue shape through and beyond an organ and its walls whereas a regular endoscopy can only view the inner surface. The procedure is non-invasive, and requires no sedatives.

There are some advantages and disadvantages to this procedure compared to others. Here is an example of Virtual Colonoscopy.

Advantages:

  • Virtual colonoscopy is less invasive, safer, and takes less time than a regular colonoscopy.
  • A thin tube to insert air into the colon is placed in the rectum rather than a long flexible tube that is moved up your colon.
  • No medicine to relax or sedate you is needed, so you will be able to drive yourself home and resume normal activities.
  • Virtual colonoscopy can show the inside of a colon that is narrow due to inflammation or an abnormal growth.

Disadvantages:

  • Virtual colonoscopy may not show polyps smaller than 10 mm (0.4 in.).
  • Virtual colonoscopy is not covered by all health insurance plans. Check with your insurance plan before having the test.
  • If a polyp is found, regular colonoscopy will be needed to confirm the diagnosis and remove the polyp so it can be looked at under a microscope.
  • This is a newer test and isn't as widely available as regular colonoscopy.
