Characteristics of Croup

Harsh Cough

Hoarseness

Respiratory Distress

What precedes croup usually?

(what do parents report in the days leading up to development of croup?)

How does it sound?

Nursing Diagnosis

Interventions

Causes of Croup

Acute spasmodic croup

Unknown

Seen in anxious or excitable children

Could be hereditary

Laryngotracheobronchitis (LTB)

Manifestations

Parainfluenza virus causes most cases viral croup

Other viral causes include:

adenovirus, respiratory syncytial virus, rhinovirus,

enterviruses, and influenza A and B

What do you suspect Tommy's diagnosis will be?

Why?

Bacterial tracheitis

Case Study

secondary bacterial infection of the trachea

i.e.: group A strep

Sore throat

What would be your highest priority nursing diagnosis?

Why? What interventions would you provide?

Agitation

Hoarseness

Use of accessory muscles

retractions

Frightened appearance

Inspiratory stridor

Cyanosis

What interventions would you expect to be prescribed for Tommy?

Tommy, age 3 is brought to the ED by his mother. She reports that he has had a low grade fever when he went to bed but woke up 4 hours later with a barky, brassy cough.

The nurse notes that Tommy has inspiratory stridor and suprasternal retractions. On further assessment, it is noted that Tommy has an inflamed mucosal lining of the larynx and trachea. He is now coughing loudly and has a hoarse voice. His mom reports that he had a runny nose beginning 3 days ago.

What education would you provide for Tommy's Mom?
