Referral Process

Learning and Support Teacher (LAST)

  • writing of the Individual Learning Plans

Long story short...

Is now:

  • assists teachers in the development and evaluation of Individual Learning Plans (ILPs)
  • assists teachers in implementation of classroom adjustments and strategies where necessary
  • coordinates execution of ILPs with Learning Support Team

Remember:

  • There's a new LST Referral Form.

  • The form is digital! You can type, save and email it to me.

  • It will be used to write the ILPs so you don't have to write the same thing twice.

  • Finished ILPs will be available on RISC or by email request to me.

Individual Learning Plans

A document of goals and learning adjustments to meet student needs (literacy and numeracy; disabilities; learning behaviour).

  • The Learning Support Referral form will be used to directly write student ILPs (saves you writing everything twice). It's important that you be detailed. Once compiled, the ILP will go through review at an LST Meeting.
  • They can be used to cover your obligations under Every Student, Every School (DEC, 2012) and Disability Standards for Education (Aust. Gov., 2005).
  • Finished ILPs then become available to all staff members in the school to access. For now, they will be available on RISC OR by email request from me.

When do we use the Referral Form?

  • Learning issues and related behaviour issues.
  • Mentoring.
  • Referral for adjustment needs (disabilities) by the classroom teacher.
  • Literacy and numeracy needs.

Evaluation

Why the new form?

  • ILPs will be evaluated every semester by the classroom teachers and the Learning Support Team.

  • More than likely, these evaluations will recommend the interventions (strategies) are working and that they be kept in place.

  • For the ILPs that aren't working, new interventions may be proposed, or the student's case may be escalated.
  • It merges many forms into one.
  • It's digital (can be completely filled out on a computer).
  • It's FOUR steps of the LST Referral Process rolled into ONE.

Things that are different...

  • New Learning Support Team referral form

  • Individual Learning Plans play a more important role and are now the responsibility of teachers to write and implement (to meet the needs of Every Student, Every School, 2012).
