In the beginning of the book the author explains Mary's back story, the fact that she isn't in the best place with the law, the fact that she is an orphan, and that she is very goth and does not like here foster parents.
When the girl is adopted she is given a new name, Dorothy. She was adopted into a sewing family with many children, all treated very poorly. She has to work everyday for her place to stay and she doesn't even get a room. She has to stay in the hall.
The new family she stays with changes her name to Vivian. She stays with them and lives a happy life, when she runs into Dutchy by chance in Minniapolis. They marry and have a child but Dutchy dies in WW2. Vivian then decides to give up there child. One day an old war buddy of Dutchy visits Vivian and Marries him. When visiting Maine they find a nice house on the river.
This is where the entire book flips. All of the sudden the story is seen from the eyes of a small irish girl in the early 1900s. This new story starts out with this girl moving to America by scraping up money from family in Ireland.
The first chapter with this new story line ends with a fire breaking out in her apartment in the slums, killing her entire family.

One night while staying at this terrible family's home, The father attempted to rape Dorothy. Instead of the father getting thrown out punished, the mother threw Dorothy out of the house. The reason she didn't have to go back to New York was that her teacher took her in until a new family could be found for her.
A couple chapters into the book the author explains that Mary had to do 50 hours of community service unless she wanted to go to juvenile detention. Her boyfriend sees a fast opportunity. His mom needs help cleaning out an old ladies massive attic. He convinces Mary to do it despite the fact that she does not like the sound of it.
This little Irish girl becomes an orphan and soon she goes to the orphanage. The life there is terrible but it does not last long when she is informed that she will be a passenger on the novel's namesake, an orphan train. It does not last long until she met a boy named Dutchy, another orphan on the train. They become close, but are separated when he gets adopted in Minneapolis and she gets adopted in a suburb.
