Strengths:

• Proven efficacy in multiple studies

• Motivated nursing leadership to adopt evidence based care

• Dedicated ICU nursing staff

• Established daily multidisciplinary rounds

• Well-integrated performance improvement methodology to the organization

Improved patient outcomes due to:

  • Reduced incidence of hospital acquired infections
  • Fewer pressure ulcers
  • Decreased risk for DVT and PE
  • Improved functional status
  • Less likely to readmit to the hospital
  • Decreased mortality rate
  • Higher patient satisfaction (Vollman, 2010)

Weaknesses:

• No medical director for the ICU making it difficult to achieve physician support

• Difficult and bureaucratic capital acquisition process making it difficult to purchase needed mobility equipment

• Physical therapy manager is territorial and unlikely to support a nurse driven protocol

• Lack of knowledge among nurses regarding safety and benefits of moving ICU patients

Opportunities:

• Identify bedside nurse and therapy champions to promote progressive mobility

• Demonstrate efficacy of protocol using a small trial with committed staff

• Partner with patient mobility coordinator to identify methods to mobilize critically ill patients without specialized equipment

Threats:

• Tight budgetary control making it difficult to receive approval for additional equipment

• Difficulty obtaining ongoing administrative support if protocol is not immediately successful
