Definitions

When?

Directly after the situation get stable within 24 hours

In case of sentinel event

Notify the in-charge or HOD

And fill out an OVR within 24 hours as soon as the situation get stabled

Objectives

Incident: is an event or happening, unusual occurrence that causes unanticipated harm to a person

Variance: any event or circumstance not consistent with the standard routine operation of the hospital and its staff or routine care of a patient/visitor

Sentinel Event: any unexpected occurrence involving death or major permanent loss of function or psychological injury, not arising from the natural course of the patient's illness or underlying condition.

Near Miss: an event or incident that could have resulted in an accident, injury or illness but did not, either by chance or through timely intervention
