Drug Classification: Antiasthmatic & COPD Preparations
Mechanism of action: Adrenergic bronchodilators and phosphodiesterase inhibitors both work by increasing intracellular level of cyclic-3’,5’- adenosine monophosphate(cAMP); adrenergics by increasing production and phosphodiesterase inhibitors by decreasing breakdown. Increased levels of cAMP produce bronchodilation.Corticosteroids act by decreasing airway inflammation.
Doxofylline's activity is due to its ability to inhibit phosphodiesterase enzymes followed by an increase in cyclic-3', 5' adenosine monophosphate (cAMP) resulting in smooth muscle relaxation.
Dosage / Direction for Use: Adult: 1 tablet 2-3 times daily. Elderly: ½ tablet 2-3 times daily.
Form: Tablet- 400 mg; syrup- 100mg/5mL
Contraindication: Hypersensitivity to doxofylline or to any of the ingredient of Doxofix or to other methylxanthines. Patients with acute myocardial infarction and hypotension and lactation.
Special Precaution: Liver disease, CHF, chronic obstructive lung disease, concomitant(naturally accompanying or associated) infections, and pregnancy since the safe use of doxofylline during pregnancy has not been established.
Adverse Reactions: Nausea, vomiting, abdominal pain, epigastric distress, insomnia, headache, nervousness and dizziness may occur. Adverse cardiovascular effects include tachycardia, extrasystoles and palpitation. Albuminuria and hyperglycemia may also occur.
Nursing Responsibilities
Assess lung sounds, pulse and blood pressure before administration and during peak of medication. Note amount, color, and character of sputum produced.
Monitor pulmonary function tests before initiating therapy and periodically during therapy to determine effectiveness of medication.
Observe for paradoxical bronchospasm (wheezing). If conditions occur,withhold medication and notify physician of other health care professional immediately
Use cautiously with patients having liver disease,chronic heart failure, chronic obstructive lung disease,concomitant infection and pregnancy.
Monitor closely for any side effects or adverse reaction to the pertaining drug. If conditions occur,withhold medication and notify physician of other health care professional immediately.
