Complications of UAC and UVC lines
- Necrotizing enterocolitis
- peritonitis
- umbilical fistula
- omphalocele
- vascular compromise of the kidneys
What are they?
Umbilical catheters that are either inserted through the umbilical vein or artery.
Our main concern as the nurse!!!
UAC- Thrombosis, Hypertension, air embolism, hemorrhage, intestinal necrosis,hypernatremia,and infection
UVC-cardiac tamponade, necrotising enterocolitis, hepatic necrosis, portal hypertension, and thrombotic endocarditis
Why are they used?
They are used if the infant is in severe respiratory distress or if they will need several blood draws within the first few days of life. They give us a rapid and reliable method for vascular access. They also give us continuous blood pressure readings.
- Constant hourly assessment of the site: Color changes above and below site, leaking of fluid around site, swelling, and if the dressing is dry and intact. Assess legs and hands for discoloration.
- Check the pump to make sure the correct rate is set. Since we are dealing with neonates it is usually a drip rate that is set on pump, so the correct rate is extremely important.
- Flush lines frequently and aspirate for blood return
- keep lines clear of blood
- Make sure and use the correct line for medications, blood, or blood draws.
UAC
UAC-goes through the umbilical artery.
- continuously monitors blood gases and arterial blood pressures
- allows access for blood draws
- fluid allowed in this line-0.45 Normal Saline with heparin or .45 Normal Saline with sodium acetate
- Caution: DO NOT USE FOR BLOOD PRODUCTS, VASOPRESSORS,or CALCIUM BOLUSES
usually only stays in place no longer than 6 days
UVC
What can we do to make it even easier to differentiate between the two?
UVC-goes through the umbilical vein
- can be used to give hypertonic solutions, medications, TPN,lipids, blood transfusions, constant monitoring of central venous pressure and vasopressors
- also used for emergency access for fluid and measurement of pH and PaCO2
- Caution: DO NOT USE FOR PLATELET TRANSFUSION
usually stays in place up to 14 days but can stay in as long as 28 days
