Objectives

-- Describe what vasoconstrictors do in the body.

-- Discuss the physical signs of too much vasoconstrictor.

-- Identify the maximum dosage of vasoconstrictor.

Vasoconstrictors

Dr. Zang, DDS

Watch Dr. Malamed's Video

on Vasoconstrictors

Avoid Epinephrine if…

Concentration of Epinephrine

Expressed as RATIO

1,000 mg:1,000 mL = 1 g/L

Amount of epinephrine per cartridge:

1:50,000 = 0.02mg/mL x 1.8mL = 0.036 mg

1:100,000 = 0.01 mg/mL x 1.8mL = 0.018 mg

1:200,000 = 0.005mg/mL x1.8mL = 0.09 mg

Know how many mg are in the cartridge you are using!

History of Stroke (Also recent MI)

Unstable Cardiovascular disease

Radiation Therapy

Brittle Diabetes (uncontrolled diabetes)

Hyperthyroidism (uncontrolled)

Tricyclic antidepressants

Non-selective beta blockers

Allergic to sulfites

(Epi. preservative =Sodium Bisulfite)

Vasoconstrictor Toxicity

Fear

Anxiety

Restlessness, tenseness, throbbing headache

Tremor, perspiration

Weakness, dizziness, pallor

Respiratory difficulties, palpitations

Bp , HR , cardiac dysrhythmias

Vasoconstrictor Maximum Dose

for Epinephrine

Maximum Recommended Dose (MRD)

MRD is the Amount of local anesthetic or vasoconstrictor a patient can be injected with before toxicity may set in.

Expressed in mg/kg or mg/lb.

*Absolute – Never ever go over this amount no matter what other factors are present.

Flat mg amount.

How does this help your patient?

Why Add Vasoconstrictors to

Local Anesthetics?

ECG

62

bpm

Vasoconstrictors

Benefits with Local Anesthetic

May the Floss

be With You!

1. Constrict blood vessels

2. Increase Heart Rate/Contraction

3. Slow absorption of local anesthetic

1. Decreases blood flow to area of LA

2. Increases LA Duration

3. Slows absorption of LA into CVS

Question:

Vasoconstrictors _________ blood flow to the local anesthetic site.

Vasoconstrictor Maximum Dosage

(MRD)

What physical signs will your patient exhibit if they are give too much vasoconstrictor?

Hint: Think "Fight or Flight."

(Write these down for class discussion.)

Vasoconstrictors....

Decrease risk of toxicity

Increase working time of local anesthetic

Hemostasis

Which of these is more important?

Common Vasoconstrictors

in Dentistry

  • Epinephrine (Most used)
  • Levonordefrin
  • (Neocobefrin)
