Background

The Issue – Concern

Lack of motivation from the nurses to properly wash their hands that results in the transmission of nosocomial infections

(MRSA & VRE)

Rational for Selection

- Hands and equipment contribute to the spread of nosocomial infections (Sax et al., 2007, p. 10-11).

- Spread of nosocomial infections is preventable, and is cause for comorbidity & mortality. (McLaughlin & Walsh, 2012, p. 653).

Current EBR QI Initiatives

- Visual cues

- Frequent reinforcement of hand hygiene

- Providing positive feedback

- Changing the beliefs and subjective norms of the unit

All improved hand hygiene compliance.

Increasing nurses’ motivation to:

1. Perform proper hand washing

&

2. Equipment sanitization

Prevent the spread of VRE and MRSA.

PDSA

Hypothetical Clinical Unit

  • Small town research hospital on a medical unit
  • East and West wing of unit
  • Half the unit staff will participate in the study
  • During the experimental phase, the nurses working on the experimental group will be followed

Barriers & Facilitators

Barriers:

  • Challenges from maintaining a consistent level of motivation in nurses.
  • Housekeeping unaware of missing soap/paper due to poor communication.
  • Inadequate knowledge/education regarding proper infection control techniques.

Facilitators:

  • Knowledge of the presence of an infectious pathogen
  • Visual cues to remind nurses to perform appropriate hand hygiene

Expected Outcome

To decrease the transmission of nosocomial infections by increasing the motivation of nurses to properly perform sanitization of hands and equipment

Strategies implemented will maintain the motivation to consistently integrate infection control measures

Reduction of spread of nosocomial infections which ultimately reduces their rate of morbidity and mortality.

Small changes in nurses' practice can have a strong impact on patient health and safety

In the end people are less likely to contract nosocomial infections, less isolation, better prognosis and therefore better load
