Case Background

Use of the Modified Vinsot Technique for a Partial Phallectomy

  • 10 y/o QH gelding referred to OSU CVHS for paraphimosis with intermittent episodes of priapism
  • Paraphimosis initially caused by injury and rDVM treated for two days after initial episode before referring

Paraphimosis with recurrent priapism

By Jason Anton

Oklahoma State University

Paraphimosis

  • The inability of the penis to retract into the preputial cavity resulting in reduced venous and lymphatic return with concurrent tissue compromise
  • Can be caused by injury, phenothiazine derivatives administration, and certain disease processes
  • Occasionally accompanied by priapism caused by the corpus cavernosum penis becoming engorged with blood

Normal Penile Anatomy of the Horse

Acknowledgements

Perineal Urethrostomy

Removal of the devitalized tissue

Placement of the Callicrate band

Potential Complications

  • Incision created on the perineal raphe, ventral to the anus
  • Urethral stoma created by suturing the urethral mucosa to edge of skin incision

Advantages

Spatulation of the urethra to the perineal skin

  • Insufficient placement of the callicrate band
  • Dehiscence of the PU site
  • Urine scalding due to lack of posturing during urination

  • Society for Theriogenology
  • OSU CVHS
  • Elimination of unecessary anesthetic risk
  • Low cost salvage procedure
  • Rapid recovery
