Humpty Dumpty

What's on the assesment?

What this means for our readiness

Why change to this assessment?

How this helps our patients

The assessment is already part of our current notes

Our current falls scale is not geared toward our patient population

Meets the Patient Safety Goal #9

Gives more detailed information on what to do for patients safety also comes with handouts for care takers and/or patient.

The Humpty Dumpty Falls© Assessment Tool is a tool that can help to predict the possibility of a pediatric patient fall.

NPSG.09.02.01

Elements of Performance for NPSG.09.02.01

1. Assess the resident’s risk for falls.

2. Implement interventions to reduce falls based on the resident’s assessed risk.

3. Educate staff on the fall reduction program in time frames determined by the organization.

4. Educate the resident and, as needed, the family on any individualized fall reduction strategies.

5. Evaluate the effectiveness of all fall reduction activities, including assessment, interventions, and education.

The tool is a cumulative calculation.

• There are 7 parameters; each parameter receives at least a minimum score of 1.

• If for some reason the items in any parameter are not applicable the child would receive the minimal score of 1.

• If a child falls into multiple categories in a parameter, the highest score of the possible choices would be given.

• Example if a child with a history of seizures (neurological) is admitted for pneumonia (alteration in oxygenation), then the neurological condition would be weighted more.

• Each parameter is added in a cumulative fashion.

• The highest score a child can receive is a 23.

• The lowest score a child can receive is 7.

• Any child with a score of 12 or above is considered “High Risk”.
