- DA Form 5987- Motor Equipment Dispatch
- DA Form 5984- Operators Permit Record
- DA Form 5988- Equipment inspection and maintenance
- DA PAM 750-8- Army Maintenance Management
- When the mission is complete, the operator preforms the AFTER PMCS.
- Annotates new faults on DA Form 5988-E
- Write down the new mileage, and how much gass was put in during the mission
- Operator turns in the Equipment Record Folder with all the forms to the dispatcher.
- Dispatcher will check forms for any new faults and update SAMS-1
- Operator must be licensed to operate the equipment either in SAMS-1 or have a valid up-to date DA Form 5984-E
- The dispatcher will give you the keys and the dispatch printout with their signature
- Sign the dispatch print out, if you have a 2ND operator they should sign under the 2ND operator
- Write down the starting mileage
- Put in the folder along with the DA Form 5988
PMCS
DA form 5988-E
- PMCS- Preventative maintenance check and services
- PMCS is requires before, during, and after each use
- Checks are done weekly, semi annual, annual, and bi- annual depending on the equipment
- PMCS reduced equipment failures, improve effectiveness, and reduces injury
- DA Form 5988 is used to write down any deficiency found
- Any faults that can be fixed at that level are repaired.
- Fault that can not be fixed are recorded and commander desided if the vehicle can be dispatched
