SHEQ Induction

Implement a Health, Safety and Environmental Management System based on the rules and regulations as laid down by the Occupational Health and Safety Act (No. 85 of 1993) and any other relevant legislation.

Identify and initiate a plan of action to eliminating or reducing risks and hazards in the workplace.

Promote and maintain a high standard of good housekeeping.

Enforce Line Management responsibility for a healthy and safe working environment.

Provide an induction

Ensure all employees who perform assigned tasks have a clear understanding of what is expected in terms of Occupational Health and Safety, in relation to that job function.

All Sub-Contractors tol comply with our policies and standards.

Create an environment that encourages a culture of personal responsibility for health and safety.

Promote open communication on health, safety and environment issues with all relevant persons on and off site

Monitor and report on the occupational health and safety performance of the company.

Occupational Health & Safety Act 85-1993 (OHSA)

16.1 - the Chief Executive Officer is accountable for the implementation of the OHSA.

Take reasonable care for the health and safety of himself and of other persons who may be affected by his acts or omissions;

When do we conduct incident investigations?

Near Miss and Minor Incidents

The Supervisor or the other designated person and SHE Representative of the area where incident occurred to initiate incident investigation within 7 days

Major Incidents and Section 24 Incidents

The Supervisor or the other designated person and SHE Representative of the area where incident occurred to complete the preliminary investigation within 3 days.

a.    any person dies, becomes unconscious, suffers the loss of a limb or part of a limb or is otherwise injured or becomes ill to such a degree that he is likely either to die or to suffer a permanent physical defect or likely to be unable for a period of at least 14 days either to work or to continue with the activity for which he was employed or is usually employed;

b.    a major incident occurred; or

c.     the health or safety of any person was endangered and where -

i.      a dangerous substance was spilled;

ii.     the uncontrolled release of any substance under pressure took place;

iii.    machinery or any part thereof fractured or failed resulting in flying, falling or uncontrolled moving objects; or

iv.    machinery ran out of control,

Incident investigation:

Is an organized and planned collection of facts — the end goals:

Determine who, what, where, and when of the near miss or accident

Develop a remedy to correct the unsafe condition, act, and work practice

Investigation offer the most potential for solving problems and reducing injuries and losses

To prevent reoccurrence of similar injuries.

NB! Never use the investigation to blame, it should be fact finding not fault finding

Make sure all chemicals are clearly marked.

Do not use chemicals if you are not trained in the proper use.

Know what chemicals are in your workplace.

Read MSDS of chemicals used to be able to know what first Aid measures and fire fighting equipment is needed should an incident occur.

Store chemical in appropriate storing facility.

Use PPE as indicated in the MSDS.

Don’t leave open containers with chemicals unattended.

Know the location of emergency showers and eyewash fountains.

Know if the chemical is flammable or not.

Don’t smoke near chemicals.

Work in a well-ventilated workplace when working with chemicals

Fire Fighting

In terms of the Common Law of South Africa, you are innocent until proven guilty

In terms of the Occupational Health and Safety (OHS) Act, you are guilty until proven innocent

Don’t put yourself in a position where you have to try and prove your innocence in terms of the OHS Act – rather just comply - it’s safer and easier !!!

Review the effectiveness of health and safety measures

Identify potential hazards and potential major incidents at the workplace;

In collaboration with his employer, examine the causes of incidents at the workplace;

Investigate complaints by any employee relating to that employee's health or safety at work;

Make representations to the employer or a health and safety committee on matters arising

Make representations to the employer on general matters affecting the health or safety of the employees at the workplace;

Inspect the workplace

Accountability: Section 16 “OSHA”

The provision and maintenance of systems of work, plant and machinery

Eliminate or mitigate any hazard or potential hazard to the safety or health of employees,

Taking all necessary measures to ensure that the requirements of this Act are complied with by every person in his employment or on premises under his control where plant or machinery is used;

Enforcing such measures as may be necessary in the interest of health and safety;Ensuring that work is performed and that plant or machinery is used under the general supervision of a person trained to understand the hazards associated with it and who have the authority to ensure that precautionary measures taken by the employer are implemented; and

The purpose of reporting is:

To inform the management and ensure that incident

are investigated and that a root cause is identified.

To ensure that an injured person is treated or receive

help as soon as possible.

To report to workman’s compensation commissioner through Occupational Nurse for payment of any medical expenses.

The purpose of incident investigation is:

To record the findings and causes, and to make recommendations

To prevent recurrence –this Reducing company loss and increase the profit.

Identify what is burning: Choose the right fire extinguisher for the job.

BLUE LABEL - DCP (dry chemical powder) or STP for any type of fire

BLACK LABEL - CO2 for electrical fires

Check pressure gauge only on extinguishers with a blue label. If the needle is on:

RED: Under pressurised or dead - CANNOT BE USED

GREEN: 100% in working order - CAN BE USED

TEST fire extinguisher BEFORE proceeding to the FIRE VICINITY.

Safe handling of chemicals

Van Dyck Carpets objectives are discussed, agreed on, maintained at management level and reviewed regularly at management meetings to ensure objectives are being achieved. The policy and requirements are communicated to all employees.

OHSE Policy

All contractors on site including visitors - to be inducted.

Each Manager is responsible for visitors visiting each department.

Security/designated member will induct all contractors on site.

SHEQ department will induct all new staff.

Contractors to ensure the following minimum documents are submitted and approved before awarded a tender:

Proof of Registration with WCA (Letter of good standing).

Risk assessments of tasks to be conducted at Van Dyck.

Competent person – Certificates/ other proof of competency.

Appointment letter of Supervisor who will be based on site.

All inductions will be valid for a period of one year

Proof of induction in a form of register to be submitted and kept by SHEQ department.
