The Bossy Baby Project

redefining oral feeding success in a Level III NICU

Dr Lindsay Meyer Turner, PhD, CCC-SLP

What is your definition of a SUCCESSFUL oral feeding?

Introduction & Literature Review

Current research shows that infant directed feeding programs are more successful and developmentally supportive for premature infants

Shorten's Length of Time to Attain Full Feedings

Fewer Adverse Events

Better Weight Gain

Changing our Mindset

The goal is NOT to complete a feeding, it is for the infant to have a good QUALITY feeding

Changing our Mindset

Remember: QUALITY > QUANTITY

INFANT DRIVEN > VOLUME DRIVEN

INTENSIVE care vs intensive CARE

Be mindful of your words...

  • I can feed a rock.
  • Let me try. I can get him to finish it.
  • He didn't want to take it, but I got it in him.
  • It wasn't a pretty feeding, but I got him to finish.
  • She just PO'd at 0900. She will probably be too tired to PO again at 1200.
  • He got sleepy after 10 minutes, but I got him to finish in 25 with some encouragement.

Instead let's say...

  • If he's not sucking anymore, it could mean different things. Let's see if we can figure out what he's trying to tell us.

Instead let's say...

7 Mistakes NICUs Make with Oral Feedings Every Day

Elements of Cue Based Feeding

Cue based feeding: "nipple feedings are intiated in response to infant's behavioral cues and stopped when the infant demonstrates satiation" (Tosh & McGuire, 2007)

Quality Scores (QS)

Knowing When Baby is DONE

  • Apnea, bradycardia, desaturations
  • Arching, straining, fussiness
  • Emesis

Force Feeding & Feeding Aversion

  • Difficulty feeding (e.g. transitioning to baby foods, cup and straw drinking) continues to be a significant issue that NICU graduates continue to deal with through the first year of life, according to parents (CITE)
  • Stress during oral feedings can alter sensory-motor pathways in the brain that guide the infant away from feeding (Shaker, 2013)

Long Lasting Effects

  • 54% of parents at one month PMA report a feeding problem (Thoyre, 2007)
  • Oral motor dysfunction persists beyond infancy in 33% of very early births (25.6 weeks GA) and 25% have oral hypersensitivity and behavioral feeding problems at early school age (Samra et al., 2010)
