The Case of

The Crying Baby

INTRO

Sara and Matt had a daughter named Kayle.

First few weeks of life Kayle was a thriving, beautiful baby with blue eyes. Growing at a normal rate.

At 6 weeks old, Kayle began crying all day and all night.

Doctors told Sara and Matt they were being overly anxious and overprotective new parents

At 3 months old, Kayle's symptoms and crying was getting even worse. They sought out a specialist.

DOCTOR VISITS

DOCTOR VISITS

Hubble

Doctor Hubble

  • had big background in pediatric urology

Saw the serious, ongoing symptoms

  • requested urine sample
  • sample contained an overwhelming amount of puss and blood cells

Puts Kayle on antibiotics for a urinary tract infection and requests more tests

  • test results proved Kayle's kidney function was decreased on the right and limited on the left

Hubble refers Dr. Lefttt and Dr. Wright

Where are the kidneys?

Superior to the rectum

Inferior to the heart

Lateral to the esophagus

Proximal to the liver

Superior to the coccyx

What do the kidneys do?

  • Every day, the two kidneys filter about 120 to 150 quarts of blood to produce about 1 to 2 quarts of urine, composed of wastes and extra fluid

What happens when they don't function properly?

  • puts stress on other body parts
  • cardiovascular system
  • blood isn't clean or chemically balanced

Doctor Leftt

Leftt

Takes x-rays

  • finds the kidneys to be abnormally shaped
  • concludes it is a genetic disorder

Wants to do surgery

  • re-implant four ureters higher into the bladder
  • prevents urine from going back into the kidneys
  • hopes kidneys will begin to properly function

Risks?

Risks

  • long hospital stay
  • uncomfortable
  • bruising
  • urine infections

Rates?

Rates

  • 95-98% success rate
  • Dr. Leftt is highly skilled and had been involved in cases like this before
  • no ethical problem

Pros?

Pros

  • Without surgery, Kayle's kidney would fail within a year or two
  • make it more likely for Kayle to survive
  • Prevent scarring
  • Body would maintain homeostasis easier

Wright

Feels there's no urgency for surgery

  • let bladder grow and things will work themselves out
  • use antibiotics constantly
  • weekly analysis and tests

Wright

Long-term affects

Negatives

Matt and Sara were very concerned about how different the approaches to Kayle's problems were.

Fears:

Worried the long-term use of antibiotics would make her body immune and would eventually stop working.

Will need surgery by age 8 or younger.

Not many clinical trials done because of the criticism from other doctors.

Positives

  • No surgery for Kayle
  • No long hospital stay
  • Alternative approach
  • no ethical problem

Surgical

DECISION

I choose the surgery route for Kayle due to the fact that as Kayle is approaching being 6 months old, she is already developing infections and becoming immune to antibiotics. Kayle could eventually become resistant to the medical treatment all together.

Pros: quick fix & happy, healthy baby, 95-98% chance of success

Cons: hospital stay, 5-3% chance of not going well, expensive, many future appointments
