LDL CHOLESTEROL

Low density lipoproteins (LDL) contain a higher proportion of cholesterol and protein and are considered "bad" cholesterol. It is bad because it becomes part of the plaque, which can clog arteries and make heart attacks and strokes more likely.

Inability to tolerate statins because of muscle symptoms contributes to uncontrolled cholesterol levels and insufficient cardiovascular risk reduction. Bempedoic acid, a prodrug that is activated by a hepatic enzyme not present in skeletal muscle, inhibits ATP‐citrate lyase, an enzyme upstream of β‐hydroxy β‐methylglutaryl‐coenzyme A reductase in the cholesterol biosynthesis pathway.

ISSUE APPLIED IN ECUADOR

The trial involved 2230 patients, of whom 1488 were assigned to receive bempedoic acid and 742 to receive placebo. The mean LDL cholesterol level (± SD) at the beginning of the study was 103.2 ± 29.4 mg per deciliter.

CONCLUSION

Bempedoic acid added to maximally tolerated statin therapy did not lead to a higher incidence of overall adverse events than placebo and led to significantly lower LDL cholesterol levels.
