Document on the Medication Sheet the medication administered at the bedside
The Medication Sheet, Diabetic Record sheet form shall reflect the correct Physicians’ order for medication. The nurse who administer the medication Shall document such by initiating the time on the Medication Sheet as soon as possible.
1Following schedules shall be observed in administering
medications ordered by medical staff unless otherwise
specified by the physician
OD Daily 0800 H
BID Twice daily / every 12 hours 0800 H 2000 H
TID Three times daily 0800 H 1600 H 2400 H
QID Four times daily 0600 H 1200 H 1800 H 2400 H
Q4H Every 4 hours 0800 H 1200 H 1600 H 2000 H 2400 H
Q6H Every 6 hours 0600 H 1200 H 1800 H 2400 H
Q8H Every 8 hours 0800 H 1600 H 2400 H
Bedtime At bed time 2200 H
If a prescriber’s order does not designate a route of administration, the nurse should consult the prescriber.
Likewise, if the specified route is not recommended, the nurse should alert the prescriber immediately.
Each nurse should know the common dose, maximum dose, and how to compute dosage Drug Information Manuals and the Hospital Formulary are available on each unit. If uncertain the nurse can consult Pharmacy
department.
Independent Double Checking-
Medication and its expiry must be checked three (3) times:
1. When he/she removes medication from the shelf / drawer
2. When he/she opens the packing
3. Before administration of medication to the patient.
The nurse checks the medication administration form against the client’s identification bracelet
and asks the client to state his or her name
to ensure the client’s identification bracelet
has the correct information.
