- The FAA cracked down on UPS by conducting an investigation on the company and how many hours the pilots actually fly.
- UPS cracked down on their employees as well
- Recommendation was made to Airbus that their systems are updated regularly, because if they were this accident could have been prevented.
- "The flight crew did not sufficiently monitor the airplane’s altitude during the approach and
subsequently allowed the airplane to descend below the minimum altitude without having the
runway environment in sight."
- With the factor that the crew was fatigued, the captain did not notice the altitude differnce.
- Were landing with a non-precision approach
- The longest runway, runway 6, which was the orginal runway they were supposed to land on was shut down until 5 a.m. due to maintenance.
- Crew was coming off a prevous leg, there was time in between for rest, but was cut short due to the time that leg took and an extra 30 minutes to the hotel.
- FO stated "I was out and slept today. I slept in Rockford. I slept good, and I was out in that sleep room and when my alarm went off, I mean, I'm thinkin' 'I'm so tired,'"
