This could be as a results of an accidental access. It could be as simple as overseeing a printout/screen

In 2009, an individual from

RAF Innsworth in Gloucestershire lost a memory stick with 500 highly sensitive files, containing details of individuals' extra-marital affairs, debts and drug use.

Viruses and malware - Shamoon - discovered in 2010. Steals data, and then overwrites all of the files on the computer making it un-usable.

Targeted malicious attacks - Sony PlayStation network - 2011 - DDOS!

Intentional data tampering

Hacking - In 2013, the associated press' twitter account was hacked by Syrian hackers who reported that the white house was under attack and Barack Obama was injured.

This led to a 150 point drop in the stock exchange.

Human error - accidental deletion of files, destruction of backups, destruction of equipment......

Equipment failure - physical failure of storage medium - no backups
