The City of Los Angeles' Joint Labor-Management Benefits Committee (JLMBC) and Personnel Department/Employee Benefits Division congratulate and welcome you to the City of Los Angeles. We hope this presentation will provide you with a basic understanding of the benefits available to you through the LAwell Civilian Benefits Program.

Health Care:
It is important to understand the difference between HMO and PPO plans as you explore your health plan options:
HMO-Health Maintenance Organizations (HMOs) provide healthcare through a network of doctors, hospitals and other healthcare providers. With an HMO plan, you must access covered services through a network of physicians and facilities as directed by your Primary care Physician, except for emergencies.
Health Care:
- Members access Kaiser services through Permanente Medical Groups and Kaiser Foundation hospitals located throughout the Southern California Region (in-network care)
- Out-of-network care is not covered unless it is a medical emergency
- Members not required to designate a Primary Physician (PCP), but are encouraged to do so
- Kaiser has an integrated medical recordkeeping system so member records are accessed by medical professionals throughout the organization

PPOs- Preferred Provider Organizations (PPOs) are nationwide networks of doctors, hospitals, and other health care providers that have agreed to offer quality medical care and services at discounted rates. You can use in-network providers for a higher level of reimbursed coverage,or go to a licensed out-of network provider and receive a lower level of reimbursed benefits.
- Provides a smaller, select group of physicians, hospitals and other medical service providers throughout California
- Members must work through a Primary Care Physician (PCP)
- Out- of- network care is not covered unless it is a medical emergency
- Medical records are not integrated among Full Network service providers

Dental Care Coverage
Health Care Coverage
The City provides three dental options administered by Delta Dental:
The Employee Assistance Program (EAP)
The City provides five health plans for your choosing. You can also decline health coverage through the Cash-in-Lieu option (see Cash-in-Lieu slide).
- DeltaCare USA DHMO: a dental HMO; members choose a Primary Care Dentist (PCD)
- Delta Dental PPO: provides care through a network of dentitst who have agreed to offer covered services at discounted rates
- Delta Dental Preventative only: covers preventative dental care only/ does not cover other services (i.e. fillings, crowns and orthodontia). Those who choose this option receive additional pre-tax LAwell dollars of $5/ month

The EAP Program is designed to help you manage life challenge and improve your quality of life. The Program is administered by Managed Health Network MHN, and is completely confidential and voluntary.

- Members may seek in-network as well as out-of-network services, but discounts only apply to in-network providers

- Offers 5 telephone, web-video and face-to-face counseling sessions per person, per incident, and per benefit period at no cost to you
- Services provided by licensed providers available 24/7

Life Insurance
- Vivity is a regional network of seven major hospital systems including physicians, hospitals and medical service providers offering services in Los Angeles and Orange Counties.
- Members must work through a Primary Care Physician (PCP) when seeking services
- PCPs can refer members to other specialist among the Vivity network

The City provides basic life insurance up to $10,000 at no cost to you and gives you options for supplemental and dependent life insurance. Life insurance offers important financial protection for you and your family.
Vision Care Coverage
Health and Dependent Tax-Savings Accounts
The City automatically enrolls you in basic vision coverage for you and your eligible dependents through EyeMed.
- The cost of Supplemental Life Insurance is based on your age, salary, and the cost per $1,000 of coverage.

The City provides HCFSA and DCRAs for tax savings on eligible expenses:
If you choose supplemental life insurance for yourself, you can choose to purchase insurance for your spouse/domestic partner, children or both.
- EyeMed's network has over 71,000 providers plus national retail chains such as LensCrafters, Sears Optical, Target Optical, JCPenny Optical and most Pearle Vision locations.
- All enrolled employees receive a $150 eyewear allowance (i.e. used for frames and contacts). This $150 can only be used in a single transaction.
EyeMed offers "pop-up" clinics at City offices for on site exams and delivery of eyewear.
- You can also visit out-of-network providers and still receive benefits for covered services, however you must submitting an out-of-network claim form to EyeMed for reimbursement of out of pocket costs.
- If enrolled in cash-in-lieu, you will be automatically enrolled in employee-only vision coverage at no cost.

- HealthCare Flexible Spending Account (HCFSA): set aside $300-$2,550 to reimburse yourself for eligible healthcare expenses for you and your dependents including medically necessary expenses not covered by any medical, dental or vision plan (i.e. acupuncture, hearing aids, co payments, crutches etc)
- Dependent Care Reimbursement Account (DCRA): set aside $600-$4,992 to reimburse yourself for day care expenses for your eligible dependent children under age 13 and a disabled spouse or dependent who is incapable of self-care
If you have the option to enroll in health coverage as a dependent through your spouse's or domestic partner's employer, a second employer, or through retiree health coverage from a former employer, you are eligible to opt out of health and/or dental coverage under LAwell and choose Cash-in-Lieu to receive a check from the City for each pay period.

- Your spouse/domestic partner insurance cannot be more than your total life insurance coverage (basic plus supplemental).
- Limits: If your life insurance coverage is reduced based on your age, coverage for your spouse/domestic partner will be reduced
Accidental Death and Dismemberment Insurance
(AD&D Insurance)
2017 Health Coverage Costs Per Pay Period (Every two Weeks)
AD&D Insurance is available at an additional cost, which pays a benefit to you if you suffer a covered loss or to your beneficiary if you die in an accident. You can purchase AD&D insurance for yourself only, or for you and your family.
Deferred Compensation Plan
The Deferred Compensation Plan plays a vital role in saving money and creating future retirement income security. This plan is voluntary and supplemental to you primary City Retirement Plan. Here are the two resources for creating retirement income security:
- If you want coverage for yourself, you can choose any amount between $50,000 and $500,000. (AD&D pays a percentage for your injuries)
- In the case of an accidental death, AD&D pays 100% of your coverage amount, plus an additional $3,000, up to a maximum of $503,000

LOS ANGELES CITY EMPLOYEES RETIREMENT SYSTEM (LACERS)
- Benefits determined based on factors such as how long you work for the City, when you were hired, and your salary near retirement
DEFERRED COMPENSATION PLAN
- Benefits are based on the total balance (contributors + earnings) you accumulate in your account
- you can begin drawing on your balance when you retire
***The Plan offers a loan program, which allows you to borrow from your account up to certain limits and then pay yourself back***

Basic and Supplemental Disability Coverage:
The City provides basic disability coverage in the event of a qualified disability, at no cost to you.

- PUT AT END You may also purchase Supplemental coverage pays a higher monthly benefit on basic coverage, and pays beyond the 24-month limit if you remain disabled. (cost may increase based on age and or annual salary at the time of enrollment)
- For short-term disability (STD) benefits; in your first 180 days of disability after exhausting 100% and 75% of sick leave, you are disabled if you are unable to perform with reasonable continuity the materials and duties of your own occupation because of sickness, injury or pregnancy, or you are working and unable to earn more than 80% of your pre-disability earnings because of sickness, injury or pregnancy.

PARKING SPENDING ACCOUNT (PSA)
TRANSIT SPENDING ACCOUNT (TSA)
- Allows you to set aside up to $255 per month on a pre-tax basis to pay for parking expenses related to commuting from home to work
- Allows you to set aside up to $255 per month on a pre-tax basis for public transit (bus, rail, train, subway fares)
- The City offers $50 in the form of "Transit Match" for eligible employees
TRANSIT REIMBURSEMENT PROGRAM
- For long-term disability (LTD) benefits: after you have exhausted all sick leave and been disabled for 180 days beyond the exhaustion of your 100% and 75% sick leave, you are disabled if because of physical disease, injury, pregnancy or mental disorder:
- For the first 24 months of LTD benefits:
- You are unable to perform reasonable continue the material duties of your own occupation, or
- You are working in your own occupation and unable to earn at least 80% of your pre-disability earnings.
- After 24 Months:
- You are unable to perform with reasonable continuity the substantial and material acts of any gainful occupation for which you are reasonably fitted due to physical disease, injury, pregnancy or mental disorder.
- Provides up to $50 per month to those using public transportation to commute to work
- Assists employees in creating vanpools to facilitate carpooling as a means of commuting to work (approx. 90 vans are currently operating)
- The City provides employee parking at many City-owned and leased lots near primary City work facilities.
BIKE/WALK TO WORK PROGRAMS
