Automatic 2's and 3's

Basic Rules

1. Interpersonal problems must be scored a 2 if there

is a personality disorder diagnosis.

2. Trauma must be scored a three if there is a diagnosis of PTSD.

3. Physical/Medical must be at least a 2 if they wear glasses or have diabetes, hypertension, any CHRONIC medical condition.

4. Employment must be a 3 if they are under the age of 65 and do not work, regardless of receiving SSI.

Scoring is about what the individual needs (think about when they came into services)?

You can't describe the need for ongoing supports if you say there is no need (0 or 1) because of ongoing supports.

30 day window--NOT did it happen in the last 30 days, but WAS IT RELEVANT in the last days?

5. DECISION-MAKING: If a client comes in once a month for skills training this should be a 2; if they come in more than once a month this should be 3 because they are relying on the case manager to help process information and learn more adaptive coping skills because the client is unable to process information and think through decisions independently.

6. Legal should be a 2 if they are on probation of parole.

What does a

0 or 1

look like???

7. Living Skills should be a 3 if they live in a group home, member of ACT or live with family because of symptoms.

2 Basic Rules for Scoring.....

  • Each diagnosis must be reflected in the ANSA-there can't be a 0.

Why is 0 a problem score?

0 score states there is NO evidence; no history; no problem. If they have a diagnosis of MDD, Bipolar, or Schizophrenia then there should NEVER be a 0 score because there is a HISTORY of depression or psychosis.

So, then what is a 2 or 3?

  • Score as if the person were not in our care/not on medications/or if they were to suddenly leave our services.
