Clinical Manifestations
- Not detected on Toxicology screen
- Diagnosis made on Clinical Symptoms
- Presence of Tropane Belladona Alkaloids in Urine sample by Gas chromatography-mass spectrometry
- Absorbed through GI tract.
- Gastric Lavage is Indicated in the first 1 - 2 hours.
- Repetitive dose of Activated Charcoal not indicated as GI motility reduced inducing multiple episodes of vomiting and abdominal ileus.
- Physostigmine is the Antidote.
- Unlike physostigmine, neostigmine has a quaternary nitrogen; hence, it is more polar and does not enter the CNS. Its effect on skeletal muscle is greater than that of physostigmine. Neostigmine has moderate duration of action, usually two to four hours.
- Physostigmine is to be discontinued if excessive vomiting, salivation, diarrhoea and urination occur.
- Rapid Infusion causes Bradycardia, Hypersalivation and Convulsions.
